Job Description:
Seeking a compassionate Care Coordinator to manage and coordinate care plans for clients, ensuring their well-being and satisfaction.
Responsibilities:
Coordinate individualized care plans.
Facilitate communication between healthcare providers, clients, and families.
Conduct assessments and develop tailored care plans.
Schedule appointments and follow-ups.
Monitor client progress and adjust care plans.
Provide guidance and support to clients and families.
Maintain accurate documentation.
Requirements:
Bachelor's degree in related field preferred.
Previous care coordination or case management experience.
Strong communication and organizational skills.
Compassionate and empathetic approach.
Proficiency in Microsoft Office.
