Job DescriptionBand 5, Community Dietitian. We are looking for an enthusiastic Dietitian to join our team of 26 community dietitians, assistant practitioners and admin staff working within our community services division. This post is open to applications from newly qualified and experienced band 5 Dietitians as well as Dietetic students. We will consider requests for part time or flexible working. This post is based within the Community Services Division providing nutritional care within a variety of settings including outpatient clinics, care homes and patients own homes. You will manage a diverse clinical caseload incorporating nutritional support, frailty, chronic diseases, oncology, neurology and gastrointestinal disorders. You will be encouraged to develop your skills and competence in Home Enteral Feeding with a view to supporting patients with feeding tubes in the community. You will also deliver general outpatient clinics and education sessions for patients and staff. Multidisciplinary working and a patient centred approach is essential to providing holistic care within the community setting. We communicate and integrate well with multidisciplinary teams and the Neighbourhood Integrated Care Teams across Liverpool, which include other Allied Health Professionals, District Nurses, Social Services, Voluntary services and GP's. You will have access to regular clinical supervision with senior Dietitians and our band 5 peer supervision programme involving dietitians from across Mersey Care. We have a Dietetic Preceptorship Programme to support your development as a newly qualified Dietitian to ensure you are supported in your transition as an autonomous practitioner. We also have regular professional updates and encourage staff to develop working relationships with relevant teams and staff across primary and secondary care.
