The role of a Care Home and Home Care 'Trusted Assessor’ will suit someone who has had Care Home and Home Care experience and has been involved in completing assessments for clients who require care packages within the Care Home and Home Care environment.
This position has been developed to bridge the gap between Care Home and Home Care and NHS teams to support older adults who have been assessed as requiring care packages within their own homes as well as completing assessments for Care Homes.
You will be required to go out and meet Care Home and Home Care managers and build relationships with the managers and owners to really understand the type of services offered by each provider. You will be liaising with Discharge Practitioners and the Hospital Social Work team to assess clients on behalf of the Care Home and Home Care providers in the acute setting for care packages in a care environment.
On a day to day basis, you will develop relationships with a wide range of services and individuals, such as hospital Discharge Practitioners, Social Workers, Ward staff and the providers of the Care Home and Home care services. You will be based at the Luton and Dunstable Hospital.
