We are looking for a dynamic, strong, caring dispatcher to join our team. Job description Monitor, update, and control the location and movement of field units Act as a resource point for healthcare facilities and other agencies utilizing the services of Citywide Ambulance Ascertain proper levels of response and allocate resources to match dynamic system demands Monitor and document response times of units, investigating and reporting all response time exceptions Maintain excellent knowledge of company policies, procedures, and medical protocols. Qualification: TLC Driver’s License New York State Drivers’ License Registration Card Social Security Card TLC car insurance 18 years old or older Vehicle required: Wheelchair or any vehicle Job Description: Pick up medicaid customers and drive them to and from their doctor's appointments. Pick up passengers at prearranged location, taxi stand, dedicated bus stop routes, on a hail and take the passengers to any desired location. Assist passengers with entering and exiting the vehicle. Lift, carry and manage all passengers luggage(s) and or medical device used for ambulation. Drivers are required to pick up the trips off the medicaid transportation app after they complete their current trip and call all customers to inform them of their arrival time. Driver may use any app for taxi fare as well as use any location to find customers Driver are responsible to call any customers to return any items left in car Drivers are required to clock in and out on a daily basis and start their shift from home. Safely transport all passengers from one location to the next. Follow all traffic laws and ensure there is always gas in the fuel tank. Must get fingerprints at indentogo. Able to use a GPS and pay tolls. Drivers must have excellent customer service skills, friendly. Drivers must vacuum and keep car/vehicle clean at all times. Required to perform pre-trip and post trip inspection to ensure their vehicle is safe to transport passengers. Service car as needed, get oil change, air in tires, and keep windows clean. Ensure all requirements by Taxi and Limousine Commission is met before their expiration date. Keep hair neat and wear any clothing until driver receives a uniform. If driver uses a wheelchair vehicle, driver must know how to use the wheelchair lift safely. Make change, accept payments for all fee for service trips and keep records of all trips. Drivers must be willing to accept all types of payments, ie cash, cash app, zelle, paypal etc. Drivers are responsible to pay tax as independent contractors. Drivers must contact police if any customers does not pay the driver and the driver must file a police report. Drivers are responsible for their income. Driver must pay base fees and/or taxi meter fee and/or rental fee on time every month Driver is responsible for late fee accrue on base fee, rental fee and taxi meter fee Driver must pay all tickets on vehicle and complete TLC inspection on time Driver must not loiter on Waterlily Car Service property Drivers must email all police report to Waterlily Car Service

**Part-Time Day Habilitation Driver/DSP** Qualifications •Must have at least a high school diploma or GED •Driver's License (Required) •Must be able to drive a 12-15 Passenger Van •High school or equivalent (Required) Driver for Day Habilitation/Respite Program: Monday-Friday 7-10am / 1:00pm-4:00pm / 4:00pm-6:00pm ● Transporting Developmentally Disabled individuals to and from their residences to the Day Habilitation program in Bronx,NY using an agency vehicle ● Arrive at destinations on schedule ● Ability to plan for routes: ○ Use navigation applications to determine the best route(s) ○ Be aware of traffic, weather delays ● Ability to develop a rapport with the individuals of various backgrounds ● Must abide by OPWDD rules and regulations ● Ensure the agency vehicle is always in proper working order: ○ Fueled and ready to use ○ Report any vehicle maintenance needed to supervisor ○ Vehicle interior is kept clean ● Maintaining mileage record and Pick-up/Drop-Off logs Required Education and experience: ● Clean and valid New York State Driver’s License ● Excellent verbal and written skills ● Interact with individuals professionally and respectfully ● Ability to pass a background check ● Commercial Drivers License a plus ● Experience providing direct care of services to adults with developmental disabilities ● CPR & First Aid trained a plus as well

JOB DESCRIPTION: Direct Support Professionals work with individuals and/or small groups of Developmentally Disabled (DD) and/ or Intellectual Disabilities (DI) individuals. Provide instruction, supervision and direct assistance in facilitating services as outlined in individual’s Habilitative/ Staff Action Plan. Provide training and supervision in various activity of daily living, including: self-help, communications, interpersonal relations, leisure activities, etc. Transport program participants to and from their house and activities. Ensure that the vehicle is maintained clean and in proper working condition. ESSENTIAL DUTIES AND RESPONSIBILITIES: Assist in the development of the individual’s Habilitative/ Staff Action Plan, Safeguards and in meeting that the objectives are in their plan of services. Assistance may be required in the following areas: cleaning, preparing snacks, social skills, budgeting, reading, math, computer skills, self-advocacy skills, volunteering, recreational and physical fitness activities. Staff may need to provide hands on demonstration, modeling, prompting, and instruction in the use of these (and other) skills. Completes all required Services billing documentation in an accurate and timely manner. Submit daily service notes accurately and contemporaneously. Report and document all incidents. Ensures that individuals’ goals are person centered and promote independence, inclusion and/or integration into the community. Transport program participants to and from their houses and activities. Ensure that everyone in the vehicle is wearing their seatbelt. Ensure that the vehicle is maintained clean and in proper working condition. Maintain all transportation records required by the program. Maintain all transportation records required by the program. Inspect the vehicle on a daily basis to ensure that safe to transport staff and program participants. Serve as a liaison between the parents and the program. Other duties may be assigned with or without prior notice.
