HonorVet Technologies is seeking a travel nurse RN Case Management for a travel nursing job in Santa Rosa, California. Job Description & Requirements • Specialty: Case Management, • Discipline: RN, • Start Date: 12/01/2025, • Duration: 12 weeks, • 40 hours per week, • Shift: 8 hours, days, • Employment Type: Travel Unit Description – Case Management Department Type of Care: • Provides comprehensive case management for acute care patients in a fast-paced hospital environment., • Focuses on effective discharge planning, care coordination, and resource utilization., • Adults and geriatric patients across medical, surgical, ICU, and specialty units., • 8-hour shifts (08:00 AM – 04:30 PM)., • Schedule varies based on departmental needs., • Float and weekend coverage required (every other or every third weekend)., • Day Shift: 1:20, • Perform patient assessments and develop individualized discharge and care coordination plans., • Participate in interdisciplinary and family meetings to ensure effective communication and planning., • Collaborate with HMOs, IPAs, and community providers for placement and follow-up., • Arrange DME, SNF, rehabilitation, and ambulance transport independently., • Ensure compliance with regulations and hospital policies during discharge and transition processes., • Documentation: Epic (HARDSTOP), • Medication Management System: Pyxis, • Access to pharmacy, radiology, respiratory therapy, physical therapy, social services, and transportation teams., • Current acute hospital Case Management experience within the past 2 years is required., • Must demonstrate ability to work independently in a high-volume setting., • Experience with HMOs, IPAs, DME coordination, SNF placement, and ambulance transport required., • Minimum 2 years of acute hospital Case Management experience in care coordination and discharge planning., • Behavioral health experience alone does not qualify., • Orientation: 40 hours., • Scrubs: Provided (business casual or solid color)., • Limited pre-approved time off (1 request per contract).
