We currently serve over 300 Skilled Nursing Facilities including sub-acute, long-term care, and assisted livings throughout NJ, NY, PA, and MD. (PT and FT), • Malpractice Insurance Provided, • PTO & Holiday (Full-Time) Key Responsibilities: • Conduct individual and group psychotherapy sessions for patients in a subacute or skilled nursing facility., • Assess, diagnose, and treat a wide range of emotional and behavioral health conditions, including depression, anxiety, adjustment disorders, and cognitive decline., • Develop and implement evidence-based treatment plans tailored to the needs of each patient., • Collaborate with facility staff, interdisciplinary teams, and family members to support patient care., • Maintain accurate and timely clinical documentation in accordance with regulatory standards using our clinician-designed electronic medical/health system (EHR)., • Monitor patient progress and adjust treatment plans as needed. Requirements: • Active and unrestricted license to practice as a Licensed Clinical Social Worker in State applying for., • Effective oral and written communication in English, • Basic proficiency with technology, including electronic health records (EHR)., • Strong organizational and documentation skills, with attention to regulatory compliance., • Ability to provide in-person services at assigned facilities, up to 45 minutes., • Ability to walk, stand, and move between patient rooms and offices within the facility throughout the day., • Full Vaccination and Booster Status may be required in some facilities. Preferred Qualifications: • Previous clinical experience in a subacute, long-term care, or geriatric healthcare setting., • History of treating Anxiety, Depression and Adjustment Disorders., • Psych or Addictions experience is a plus., • Excellent time management and organizational skills.
