The case manager's primary goal is to work with fathers by providing regular contact to facilitate program retention, completion, and access to needed support services with greater focus on economic stability and job-focused services with a clear and unwavering commitment to promoting responsible parenting and healthy marriage/relationship. SPECIFIC RESPONSIBILITIES 1. To provide systematic, coordinated outreach, intake, and comprehensive personal assessment services for all eligible participants referred by community-based agencies or identified through extensive outreach efforts, 2. Provide case management services to participants via phone, in person and/or virtually. Participants must receive a minimum of 8 case management service contacts, 3. Develop a targeted individualized case management plan with clients to identify their short- and long-term goals and assist them with their immediate including coordination of services and support as needed., 4. Provide direct services, including support, education, and focused external referrals to other needed services such as housing, substance abuse treatment, mental health programs, etc, 5. Convene weekly with case management team to review caseload, client progress, identify any barriers that could impact program retention/completion or any other programmatic challenges and successes., 6. Intensive focus on economic stability by coordinating needed economic stability supports, identifying obstacles and providing hand-on support to improve their readiness for job seeking and employment including follow-up on participants' progress regarding job placement, retention, and/or training., 7. Work as a part of a team to deliver a range of community outreach and engagement activities to promote services offered through the LIFI program to prospective partner organizations including but not limited to probation/parole, Drug and Family Court, CPS, homeless shelters, transitional and recovery housing facilities, 8. Methodically track and maintain records of all client contacts, goal objectives, progress and milestones including employment and education plans in nFORM. Ensure all entries are accurate, detailed and entered in a timely manner., 9. Participate in all professional development opportunities including but not limited to on-going training at conferences and webinars addressing Fatherhood, Healthy Relationships, Domestic Violence and Economic Stability issues., 10. Assist with the facilitation with nFORM surveys if needed, 11. PRIMARY RESPONSIBILITIES: Management of Recurring & Demand Services: • Ensure all services to be performed are sent to contractor partners (CPs) per scheduling cycle., • Ensure work order listing is sent out to the CPs per the scheduling cycle., • Ensure all callbacks are completed in a timely manner., • Track all complaints, no-shows, and service failures to resolution using RTS process in WAVES. Assign RTS to field or Contractor relations team members when appropriate., • Provide timely responses to client inquiries/issues. Responses to clients should be within an hour for hot escalations and before the end of the day for standard matters., • Ensure recurring services are scheduled within scheduling cycle. You will receive a retention bonus of $3,000 after completing 6 full months of full-time employment in this role, and an additional bonus of $5,000 after completing 12 full months of full-time employment in this role. DUTIES AND RESPONSIBILITIES: • Director to manage and direct all administrative, fiduciary and program services to ensure conformance with program objectives, agency policies and procedures, and compliance with regulatory, funding and governing bodies, including but not limited to:, • Oversee the admission of all residents to ensure adequate services, • Provide supervision to management staff and program staff, • Develop programs for clients, • Oversee quality assurance and improvement process, • Manage the operation of the program in accordance with the contract and budget, • Preform all other supervisory functions which impact the quality and productivity of the program, • Provide oversight to all residential properties including obtaining properties, lease and property management., • Participate in the recruitment, hiring, training and ongoing evaluation of staff. Provide oversight of staffing patterns of the programs., • Provide ongoing structured supervision to Assistant Directors and Managers and ensure that all staff are receiving adequate training and supervision., • Assure that consumers receive services appropriate to their needs., • Participate in the implementation of program policies and procedures in conjunction with the Vice President and Quality Assurance., • Participate in the development and implementation of program policies and procedures in conjunction with the Vice President., • Responsible for routine monitoring of all client records to ensure that records are up to date and include all required documentation in compliance with applicable individual program regulations and policy manuals., • Provide direction and support to staff during crisis situations., • Ensure that all incidents are reported in a timely manner in accordance with applicable regulations., • Ensure that Quality Assurance standards are met or exceeded in cooperation with the Manager of Quality Assurance and Vice President., • Represent the agency in facilitating the goals of the program. Represent the agency at community meeting and functions as needed., • Oversee the Facilities Team and all Agency sites to ensure sites are safe and meet the needs of the residents and staff. Track task completion for the agency., • Identify and implement quality improvement trainings and activities as part of professional development goals, • Encourage quality improvement processes and provide consistent resources for QI and professional development, • Ability to cultivate a culture of inclusion for all employees that respects their individual strengths, views and experiences.
