Responsibilities include: • Evaluate patients and develop individualized treatment plans, • Deliver skilled physical therapy services in the home setting, • Build meaningful patient relationships through one-on-one care, • Complete documentation accurately and on time, • Coordinate schedules and communicate with the clinical support team, • Drive clinical initiatives focused on delivering exceptional patient outcomesWhat You'll Bring We’re looking for clinicians who are confident, compassionate, organized, and motivated by independence and patient impact.
