Responsibilities Atlanta Medical Center is currentlyseeking a Diabetes Clinician to join their amazing Team! TheDiabetes Clinician is responsible for coordination of patientdiabetes self-management education and support through clinicalassessment, education and documentation throughout all phases ofdiabetes related care. The Diabetes Educator is responsible forproviding patient education with an understanding and awareness ofage specific needs. Assesses the biological, emotional,developmental, psychological, nutritional and patient/familyeducation needs in providing care to the patient and family.Functions as a member of the diabetes management team along withthe patient, physician, other clinical colleagues, family andadministrative staff. Education responsibilities may includeinpatient and outpatient setting and may occur in an individual orgroup environment. Serves as a diabetes resource internal andexternal to the organization. The Diabetes Educator is able toindependently perform job functions. Participates in WellStar andcommunity events (e.g. ADA walk, Diabetes Day, Health and BenefitFairs, etc.). Provides presentations to WellStar departments, civicand community partners. Qualifications Required Minimum Education *Bachelor's degree or higher in Dietetics or Nursing from anaccredited institution/program.
