Qualifications ● Baccalaureate or BSW degree ● At least one year’s experience working within the Social Services field ● Ability to engage clients and conduct psycho-social assessments and treatment plans ● Knowledge of Microsoft Word, Outlook and Excel ● Excellent writing skills Additional Desired Qualifications ● Group work experience with substance abuse or mental health conditions ● Good diagnostic ability and familiarity with DSM-IV or V ● Bilingual (Spanish-English) Job Description ● Carry an active case load of 20-30 clients ● Interview prospective clients and assess clients’ appropriateness for residency at George Daly House ● Develop treatment plan (which includes long and short term goals) for clients including psycho-social assessment, entitlement eligibility, need for medical and mental health assessment and treatment, and permanent housing ● Monitor clients’ progress in goal attainment and assist clients to redefine goals where appropriate ● Maintain up-to-date client files with necessary bibliographic and emergency information, all required DHS forms and progress notes ● Maintain up-to-date client files in CARES including progress notes, psychosocial, and income information ● Submit all paperwork necessary for housing applications ● Escort clients to housing, HRA and or medical appointments as necessary ● Provide aftercare as needed for clients for at least three months after time of placement in permanent housing ● Develop and conduct at least one group project in an area of interest (e.g., transitions, empowerment group, financial management, resident newsletter, etc.) ● Other tasks as assigned ● Participate in weekly supervision, departmental staff meeting and case conference

Minimum Qualifications • One year experience as a cook working in an institutional kitchen., • Experience in preparing food and cooking meals for large groups (88+ persons), kitchen operations, and handling of kitchen equipment., • Applicant must have successfully completed the Health Academy Food Sanitation Course., • Applicant must be willing to work a schedule that includes one weekend day weekly. - Applicant must also be free of any contagious or communicable diseases (e.g., tuberculosis).
