The Coordinator will be working closely with the Director, Signature Events, the Coordinator will develop budgets, strategies and tactics to fulfill these goals. Contacts a new member’s Primary Care Physician for other diagnose which may qualify the member for SOURCE if the new member does not have triggers to qualify him or her for SOURCE. Attends all Medical Director’s weekly meetings to discuss and sign off on the Appendix F. Enters the Medical Director’s weekly case note in Therap. Obtains the Appendix Fs from the RN and faxes them to the Department of Community Health. Reviews the members’ functioning and health status with their Primary Care Physicians to ensure concurrence with Material Data Systems or information. Other duties as assigned.
