Responsibilities ● Identifies and discriminates between physical and psycho-social signs and symptoms. Selects and performs those therapeutic measures that are essential for the effective execution of the nursing regimen for assigned patients/clients. ● Develops, implements, and continuously evaluates the nursing regimen for assigned patients/clients. ● Records and maintains nursing care plans and progress notes on patients/clients to ensure continuity of care. ● Participates in development and maintenance of a system of evaluation of nursing care, practices and procedures, including evaluation of nursing personnel. ● Interprets philosophy and objectives of the agency and Nursing Department to patients/clients, families, and other groups. ● Maintains professional competence through participation in continuing education and other appropriate learning experiences. Qualifications ● For individuals holding an unrestricted registered professional nursing license BEFORE July 1, 2020 in New York State, another state or United States territory: ● A valid New York State license and current registration to practice as a registered professional Nurse in New York State; and ● Valid and current certification in Basic Life Support (BLS) through the American Heart Association (AHA). For individuals issued a registered professional nursing license on or AFTER July 1, 2020, based on the expected implementation of Chapter 502 of the Laws of 2017 and Chapter 380 of the Laws of 2018: A valid New York State license and current registration to practice as a Registered Professional Nurse in New York State; and a. A Bachelor’s of Science degree in Nursing or higher (or other nursing degree, as identified by NYS Education Department including Master of Science in Nursing, Valid and current certification in Basic Life Support (BLS) through the American Heart Association (AHA). WE ARE HIRING RESIDENTIAL The Direct Support Professional (DSP) provides assistance to participants who need help with daily living skills, and other activities such as, activity development and training in social interaction. This position assists participants with bathing, feeding, toileting, laundry, and other self-care responsibilities. The DSP is responsible for safely accompanying participants to community resources, medical appointments, and other approved activities. The position is responsible for completing progress notes and making appropriate entries into the daily log book. FULL-TIME SHIFTS M-F 3PM-11PM, 4PM-12AM, 12AM-8AM, SAT AND SUN, 8AM-8PM, 8PM-8AM Medical Assistant/Direct Support Professional DRIVER LICENSE REQUIRED This position performs the duties listed under the DSP position in addition to assisting the participant with achieving and maintaining good health, provide support and help the participant achieve their goals in life, and live as independently as possible. They would also be responsible for scheduling and escorting the participants to medical appointments and acting as an advocate and liaison while on appointments. Administer medication, check MAR sheets and maintain participant medical records in compliance with OPWDD standards. Prepare case notes, read and make appropriate entries in facility logs daily. Review communication logs, Medication Administration Log and Medical Log at beginning of each shift. Participate as a member of the Interdisciplinary Team in the development, implementation and evaluation of the residents’ Plan of Care. Accompany participants to Cardio Club and participate in activities and carry out fire drills, evacuations and emergency procedures.
