Full job description Chai Care is opening a brand new Urgent Care Facility in Flatbush Brooklyn! At Chai Care, kindness is at the heart of our mission and we aim to serve to the community in the best way possible. We are looking for a Physician Assistant or Family Nurse Practitioner to be responsible for examining patients, prescribing medicines, and ordering diagnostic tests as needed. The PA/FNP will work under the supervision of the Physician. Consulting with Physicians when necessary. To be a successful Provider at Chai Care, you should have previous experience working in an Emergency Room or Urgent Care setting. Must be able to examine patients, diagnose injuries and illnesses as well as provide treatment. You should demonstrate strong attention to detail, excellent communication, and interpersonal skills. Physician Assistant Responsibilities: Interview, examine, and diagnose patient’s injuries and/or illnesses. Order and interpret diagnostic tests. Prescribe medications and suggest lifestyle changes. Review and maintain patient’s medical histories, records and record patients' progress. Provide documentation for insurance companies. Educate and counsel patients and their families. Research the latest treatments, innovations, advances, and trends in the medical industry. The Family Care Liaison responsible for ensuring that all program and environmental requirements related to the Family Care home are met and maintained. Acting as a direct link between the Family Care provider and the sponsoring agency, the liaison addresses fiscal and provider needs while enhancing the social functioning of Family Care providers and individuals in the home. Kindly note that this position is exempt and not-eligible for overtime pay for hours worked more than 35 during a single calendar week (Monday-Friday). General Responsibilities: - Enhances and maintains the social functioning of Family Care providers and individuals in the home. - Ensures compliance with environmental and safety standards outlined in the Family Care Manual and the New York Codes Rules and Regulations (NYCRR) that govern the Family Care Program. - Develops the Residential Habilitation Plan of Care with the Family Care provider and the individual consistent with the Individualized Service plan (ISP). - Regularly assess the Residential Habilitation Plan with the Family Care provider, ensuring service delivery by signing Form FC-DOC-4 "Statement of Affirmation for Provision of Family Care Residential Habilitation Services" (ref. Policy 10.8.1 Family Care Manual), with more frequent reviews if needed. - Prepares the plan of corrective action and ensures that the Family Care provider receives a copy as well as confirming that all deficiencies are completed In a timely manner. - Ensures that all adult residents undergo checks with the State Central Register and are fingerprinted by the Division of Criminal Justice Services. - Visits the Family Care residence periodically, specifically during each five-day period when substitute provider services are in effect, as outlined in Family Care Policy 10.8.6. - Ensures accurate recording of the activity code Family Care Residential Habilitation (FCHR) for each Family Care home the individual resides in during the month. All movements should be precisely logged into the Tracking and Billing System (TABS). Professional Conduct: - Abides by all agency policies outlined in the employee manual. - Maintains absolute confidentiality regarding all people served and their personnel records. Skills Required: - Ability to follow and carry out written and oral instructions. - Effective communication skills, both oral and written. - Tactful and effective in dealing with various stakeholders. - Experience working with individuals with developmental disabilities. Education Requirements: - Bachelor's degree. - Experience in human service programs or experience with non-profits.
