JOB DESCRIPTION: Direct Support Professionals work with individuals and/or small groups of Developmentally Disabled (DD) and/ or Intellectual Disabilities (DI) individuals. Provide instruction, supervision and direct assistance in facilitating services as outlined in individual’s Habilitative/ Staff Action Plan. Provide training and supervision in various activity of daily living, including: self-help, communications, interpersonal relations, leisure activities, etc. Transport program participants to and from their house and activities. Ensure that the vehicle is maintained clean and in proper working condition. ESSENTIAL DUTIES AND RESPONSIBILITIES: Assist in the development of the individual’s Habilitative/ Staff Action Plan, Safeguards and in meeting that the objectives are in their plan of services. Assistance may be required in the following areas: cleaning, preparing snacks, social skills, budgeting, reading, math, computer skills, self-advocacy skills, volunteering, recreational and physical fitness activities. Staff may need to provide hands on demonstration, modeling, prompting, and instruction in the use of these (and other) skills. Completes all required Services billing documentation in an accurate and timely manner. Submit daily service notes accurately and contemporaneously. Report and document all incidents. Ensures that individuals’ goals are person centered and promote independence, inclusion and/or integration into the community. Transport program participants to and from their houses and activities. Ensure that everyone in the vehicle is wearing their seatbelt. Ensure that the vehicle is maintained clean and in proper working condition. Required to perform pre-trip and post trip inspection to ensure their vehicle is safe to transport passengers. Service car as needed, get oil change, air in tires, and keep windows clean. Ensure all requirements by Taxi and Limousine Commission is met before their expiration date. Keep hair neat and wear any clothing until driver receives a uniform. If driver uses a wheelchair vehicle, driver must know how to use the wheelchair lift safely. Make change, accept payments for all fee for service trips and keep records of all trips. WE ARE HIRING RESIDENTIAL The Direct Support Professional (DSP) provides assistance to participants who need help with daily living skills, and other activities such as, activity development and training in social interaction. This position assists participants with bathing, feeding, toileting, laundry, and other self-care responsibilities. The DSP is responsible for safely accompanying participants to community resources, medical appointments, and other approved activities. The position is responsible for completing progress notes and making appropriate entries into the daily log book. FULL-TIME SHIFTS M-F 3PM-11PM, 4PM-12AM, 12AM-8AM, SAT AND SUN, 8AM-8PM, 8PM-8AM Medical Assistant/Direct Support Professional DRIVER LICENSE REQUIRED This position performs the duties listed under the DSP position in addition to assisting the participant with achieving and maintaining good health, provide support and help the participant achieve their goals in life, and live as independently as possible. They would also be responsible for scheduling and escorting the participants to medical appointments and acting as an advocate and liaison while on appointments. Administer medication, check MAR sheets and maintain participant medical records in compliance with OPWDD standards. Prepare case notes, read and make appropriate entries in facility logs daily. Review communication logs, Medication Administration Log and Medical Log at beginning of each shift. Participate as a member of the Interdisciplinary Team in the development, implementation and evaluation of the residents’ Plan of Care.
