Description Position Overview: The Life Coach at Lee Goodwin Residence provides comprehensive case management, outreach, and counseling to the residents at our supportive housing facility for young adults aging out of foster care between the ages of 18-28 year olds. The Life Skills Coach will provide referrals to: education and career programs such as: HSE, Vocational Training, Employment, and Internships. As well as: Substance Abuse Treatment, and Mental Health Services. Salary: $48,000 to $52,000 (base salary commensurate with education, training and related work experience) Responsibilities: • Conduct a comprehensive intake assessment for all new residents and review the agency’s policies and procedures with them. Develop treatment plan for all residents. Monitor clients’ emotional, social and physical well-being, as well as their progress • Maintain concise and accurate resident files in Salesforce database • Monitor residents’ medication • Provide residents with necessary referrals, including emergencies such as hospitalization, detox, etc. Inform residents about on- site, agency and community resources that are available to them. • Participate in case conferences, staff meetings and training sessions and represent the agency at assigned meetings and conferences. • Input data in Salesforce data base daily for all of the residents. • Attend professional development trainings, supervision meetings, case conferences, and staff meetings. • Provide monthly statistical reports on time and accurately. • Work cooperatively with staff at all levels to ensure quality service provision and the development and improvement of team performance. • Assist residents in the planning, prioritizing and long-term goals and short-term goals. • Maintain and oversee documentation of case files for compliance with regulatory agency. Must be able to drive a 12-15 Passenger Van. 30 Hours a week. Monday-Friday 7am-3pm (3 days a week) Duties include the following: (other duties may be assigned) 1. To assist in developing, coordinating and implementing curriculums and goals related activities by following the program’s schedule for each Individual and accompany participants safely to community resources (stores, places of worship, Library, Park, Museum, Grocery store, recreation and health facilities etc.) in order to promote the usage of available ordinary community resources. 11. To maintain daily documentation, to clock in/out daily, and to complete other Day Habilitation Documentation as required by the Program and the Supervisor. 12. To meet the deadlines assigned by the Program Supervisor, to ensure the documentation completed properly and submitted on time. 13. To maintain in the program a healthy and positive environment, including but not limited to safety, program/vehicle cleanliness, and maintenance of program supplies. Assist the driver in maintaining a safely operated bus (making sure Participants are properly seated and buckled up during transit) 14. Monitor riders’ behavior during trips and report any sign or symptoms of illness or adverse medication reaction to the site supervisor in order to implement an appropriate plan of action. 15. WE ARE HIRING RESIDENTIAL The Direct Support Professional (DSP) provides assistance to participants who need help with daily living skills, and other activities such as, activity development and training in social interaction. This position assists participants with bathing, feeding, toileting, laundry, and other self-care responsibilities. The DSP is responsible for safely accompanying participants to community resources, medical appointments, and other approved activities. The position is responsible for completing progress notes and making appropriate entries into the daily log book. FULL-TIME SHIFTS M-F 3PM-11PM, 4PM-12AM, 12AM-8AM, SAT AND SUN, 8AM-8PM, 8PM-8AM Medical Assistant/Direct Support Professional DRIVER LICENSE REQUIRED This position performs the duties listed under the DSP position in addition to assisting the participant with achieving and maintaining good health, provide support and help the participant achieve their goals in life, and live as independently as possible. They would also be responsible for scheduling and escorting the participants to medical appointments and acting as an advocate and liaison while on appointments. Administer medication, check MAR sheets and maintain participant medical records in compliance with OPWDD standards. Prepare case notes, read and make appropriate entries in facility logs daily. Review communication logs, Medication Administration Log and Medical Log at beginning of each shift. Participate as a member of the Interdisciplinary Team in the development, implementation and evaluation of the residents’ Plan of Care.
