Skills / Ability: Possess knowledge of community resources, Community-Based Organizations and private providers of services to optimize the functions of individual eligible to receive management services. Maintain face-to-face contact at least once per month with consumers. Assume responsibility for the maintenance and auditing of all assigned consumer’ Case Management records.
