Home Care Aide - Personal Care Assistant (PCA

Health Plus Management (HPM) is a Physician Support Organization and market leader in clinical practice management in the physical medicine and rehabilitation market with 50+ locations serving Long Island, NYC, Westchester, Upstate NY, NJ and Connecticut. HPM provides independent physician practices specializing in musculoskeletal care with professional business development, marketing and back-office support typically seen in much larger organizations, thereby enabling clients to start and/or grow their private practice beyond what might otherwise be feasible (while maximizing clinical service and patient care). Health Plus Management is seeking an energetic, patient-centric and detail-oriented Front Desk Coordinator to greet patients, schedule appointments, answer phones and manage administrative records. In this role, you will set the stage for each patient's appointment. You will build and maintain professional relationships with our key stakeholders - specifically our patients, office staff and clinical team, while simultaneously ensuring that the office is organized and runs smoothly. Responsibilities: • Make sure patients feel welcome as soon as they walk in the door by delivering quality and compassionate care, • Answer incoming calls, take messages, and complete outreach to patients in a courteous and professional manner, • Be responsible for obtaining and filing patients' charts, and assisting our patients as needed, • Maintain the daily operations of the office by following standard operating procedures and guidelines, • Contribute to team effort by smoothly transitioning into daily roles as needed, • Protect patient's privacy by maintaining confidentiality of personal information (HIPAA compliant), • Navigate through NextGen to access and provide patient information, • Maintain compliance by contacting patients to confirm appointments, • Respond to inquiries, resolves risen issues, collect co pays and schedule patient appointments, • Responsible for the opening and closing procedures of the office as needed, • Assist in comforting the patients and maintaining the reception area, • Other administrative tasks as needed Knowledge and Experience • 1+ years of relevant medical administrative experience, preferred, • Experience with medical insurance/verification is a plus, • Knowledge of medical terminology & procedures, and the ability to intelligently and confidently converse with patients and referring doctors regarding treatment is preferred, • Experience with EMR, preferred, • Strong computer knowledge and windows program including Microsoft word/excel Skills and Abilities • Bilingual (English/Spanish) required, • Excellent verbal and written communication, • Ensuring a great patient experience by delivering outstanding customer service, • Exceptional organizational and time-managements skills - the ability to multi-task is a must, • Adaptability and flexibility while working in a fast-paced environment, • Problem-solving ability and aptitude, • Outcome-focused, with an ability to work under pressure, • A solution-oriented mindset, • A strong sense of urgency and focus in accomplishing tasks Physical Requirements: • Prolonged periods of sitting at a desk and working on a computer., • Must be able to lift up to 25 pounds at times. Schedule: • Monday - Thursday, 11am - 8pm & Saturday 9am - 1pm Pay: $18/hour

Essential Duties and Responsibilities: The role of a Care Manager is to be responsible for the overall coordination of services to meet the goals of the client/families focused plan of care. This is accomplished by building relationships with the caregiver(s) and care team and acting as a liaison to other agencies and ensuring they receive all necessary services and implementing the plan of care. Our aim is to provide the highest quality of care management services by complying with regulatory guidelines and meeting agency standards. • Assists families to plan and obtain services appropriate to the needs of the child and family., • Completes the Child and Adolescent Needs Assessment (CANS) to identify child’s medical, social, educational and psychological needs. Then develops a comprehensive plan of care based on the identified needs., • Coordinate and monitor the delivery of identified services and the proper use of resources., • Deliver at least 2 client-centered health home services to each client on a monthly basis., • Attends and participates in continuing education, ongoing staff in-services and quarterly meetings., • Responding to feedback and corrections from company quality assurance in a timely fashion., • Perform related duties as assigned by the Supervisor., • Maintain compliance with all company policies and procedures., • Bachelor's degree is required, preferred fields of study are Psychology, Sociology, Social Work, or Human Services; or a minimum of 2 years of job-related experience providing case management or medical focused care coordination services to individuals with special needs., • Must be in-person in either our Dyker Heights or Manhattan office, or meeting with clients., • Each case requires two monthly in-home visits., • Must attend all weekly in-person staff meetings., • Excellent verbal and written communication skills, including ability to effectively communicate with clients, and healthcare providers., • Need to use electronic health record systems, databases, and other computer-based tools to manage client information and maintain accurate records., • Strong organizational skills to help manage multi-family case load., • Willingness to meet with families in their homes or communities., • Bilingual in Hindi is required., • Computer proficiency (MS Office- Word, Excel and Outlook)., • Valid NYS issued ID or driver’s license; must be willing to travel throughout the 5 boros of New York City., • Ability to travel to numerous locations to meet with clients, attend meetings and visit healthcare facilities., • Ability to safely and successfully perform the essential job functions consistent with the ADA, FMLA and other federal, state and local standards, including meeting qualitative and/or quantitative productivity standards., • Ability to maintain regular, punctual attendance consistent with the ADA, FMLA and other federal, state and local standards., • Must be able to lift and carry up to 35 lbs.
