RESPONSIBILITIES: Conduct interviews (initial/ reassessment) or follow up interview and phone calls with clients. Coordinate resources to meet all case management needs of program registrants. Provide clients with educational information, case assistance, entitlement counseling referral services supportive contact, and advocacy. Maintain statistical records of client contact for annual cost reports. Under the supervision of the Program Director develop case management care plans. May include provision of short and long-term goal oriented counseling services to clients and specific concrete needs. Be knowledgeable with and entitlement programs and procedures. Maintain and complete current and accurate written records of contact with clients. Develop and maintain liaison with other agencies on behalf of the registrants. Keeping Program Director informed of any significant information or changes on behalf of the client. Responsible for periodically updating client information. Refer to and/or communicate to Program Director or/and any other disciplines any relevant information which impact as registrant. SUPERVISION RECEIVED Respond directly to the Adult Day Care Program Director. CONTACT WITH OTHERS: Regular internal and external contacts involving considerable tact and discretion. Promotes an environment of customer service, hospitality, and student satisfaction. Our Brooklyn based not for profit Community Center is currently seeking a full time Handyman. Under the supervision and direction of the Facilities Manager, the Handyman’s main duty is to assist in the overall maintenance of the building and grounds. He/she would be responsible for basic building repairs such as plumbing, carpentry, painting, plastering, electrical and HVAC and to other building related maintenance and repair. Duties: Perform repairs and standard maintenance on equipment associated with buildings, grounds, fixtures, machinery, HVAC, electrical and plumbing and work with outside vendors as necessary. Ensures that these tasks are completed and closed out in a timely manner. Perform basic carpentry, painting, plumbing and electrical duties as may be needed for the maintenance of the building and grounds and/or in support of program and service departments of the Center. Complete preventative and scheduled maintenance work orders. Provide assistance to porters and cleaners as needed. Perform other related duties, as assigned.
