Job Description JOB DEFINITION: Conduct initial screening of all potential program participants; provide overall administrative support to Project Forward staff under the supervision of the Program Director; Conduct relationship workshops using the evidence-based curricula. Responsible to engage in activities to locate, engage and intake participants. Assist to provide economic stability services to eligible participants. Maintain effective communication with participants and other service providers to coordinate program activities and ensure Individual Development Plan goals are achieved. Flexibility required for potential nights & weekends to accommodate outreach/workshops/intakes etc. EDUCATIONAL REQUIREMENTS: High school diploma or equivalent. Some college or vocational business school training preferred. EXPERIENCE REQUIRED: Experience as an Office Manager or Administrative Assistant; excellent computer skills with proficiency in Microsoft Office required; excellent oral and written communication skills; demonstrate knowledge and experience with electronic health records; strong organizational, multitasking and decision-making skills; Experience in a mental health related field, coordination of services, employment/education services, completing collateral contacts; Bilingual in English and Spanish preferred. DUTIES AND RESPONSIBILITIES: • Provide administrative/office support to the program as assigned by Program Director and/or Program Manager, as program requires., • Responsible for daily updating/maintaining referral/outreach databases. Respond to new referrals within one business day to conduct initial screening of participants to determine eligibility, need, and to schedule intake., • Assist Program Director and Program Manager with outreach & follow-up through phone calls/written correspondence, follow-up notes, notifications of changes in client's program participation etc., • Prepare and modify documents including correspondence, reports, drafts, memos and emails, • Answer incoming calls for the program and assume other receptionist duties as needed, • Assist with training staff members as directed by Program Director (i.e. nFORM entries, departmental policies & procedures, etc.), • Manage the requisition, distribution and inventory control of all Project Forward office supplies, • Complete data entry data into nFORM and EHR in regards to admissions, discharges, service assignments, documentation of participants' progress and other data elements as needed., • Schedule and participate in team meetings as well as record and distribute the minutes to team members, • Maintains training records for all Project Forward staff., • Conduct relationship workshops using evidence-based curricula. Provide support to participants around workshop content as needed both in the office and in the community., • Conduct intakes for potential participants in cooperation with Program Director and Program Manager virtually, in the office and in the community, including data collection nFORM surveys., • Ensure participants receive services/training in accordance with their Individual Development Plan - including coordination of care with other providers, create individualized employment plan (as needed), documentation of progress, goal achievement., • Identify and implement quality improvement trainings and activities as part of professional development goals, • Complete QI Training during onboarding and participate in QI refresher training as needed., • Ability to cultivate a culture of inclusion for all employees that respects their individual strengths, views and experiences.
