Job Description POSITION SUMMARY: Under the direction of the Assistant Director of Nursing or Designee, the incumbent will assist with organizing, documenting, and providing care provided to individuals who are supported by The Arc Rockland. The Medical Coordinator will report to the Assistant Director of Nursing or Designee and care delivery will be done in compliance with rules and regulations of OPWDD as well as The Arc Rockland. This position will also review medical documentation and ensure that said documentation is complete and current, including MARS, obtaining updated prescriptions, diet orders, physicals, and other medical documentation required from families/medical providers/other team members. This position is responsible for reviewing all medical charting documentation daily and following up with staff to obtain missing documentation. The Community Services Medical Coordinator must be AMAP certified and administer medication to individuals as needed. The incumbent will be required to travel to several locations weekly and may perform required transportation services. POSITION QUALIFICATIONS: Minimum Education: High School Diploma or GED/TASC required. Minimum Experience: One year of work experience with persons with developmental disabilities is preferred. Other: Must have a valid driver's license and meet Agency requirements for driving. Maintains confidentiality of organizational and individual information., • Respectful of the rights and feelings of others and maintains A demeanor appropriate to a work environment., • Reports to work and leaves work on time as scheduled., • Accepts and efficiently performs job tasks, duties, and assignments as instructed., • Ability to prioritize assignments and delegate responsibility., • Demonstrates concern and courtesy to staff and visitors., • Within three months of hire, must successfully complete Medication Administration (AMAP), Cardiopulmonary Resuscitation (CPR), First Aid, and Strategies for Crisis Intervention and Prevention – Revised (SCIP-R), as required., • Must successfully complete training on gastrointestinal nourishment assistance, i.e. caring for G tubes, as well as Diabetes Management Training, Medication Administration Training (AMAP), Colostomy Care, and any other medical training as directed within the scope of candidate’s practice. Environmental Considerations: May be exposed to a few slip and trip hazards on the floor, odors, inclement weather, medical wastes, illness, and unpredictable work conditions. May be required to work in areas within the agency, community and offsite locations that may or may not be heated or cooled. Working Conditions: Work involves regular exposure with people receiving supports. Ability to physically assist and help lift individuals. Must drive agency vehicles, including vans, to transport individuals to appointments and community outings. Corporate Compliance: Employees must comply with agency or governing body rules, regulations and policies and procedures. Any employee who witnesses or learns about a violation or possible violation of agency or governing body rules, regulations or policies and procedures is required to immediately report this to his/her supervisor or the agency’s corporate compliance officer. Community Services Medical Coordinator SPECIFIC JOB DUTIES AND RESPONSIBILITIES The following statements are intended to describe the general nature and level of work being performed. Employees may be required to perform other job-related duties as instructed by the Supervisor, subject to reasonable accommodations. ESSENTIAL FUNCTIONS 1 Participate in medication management, including, but not limited to, administration of medication, maintenance of MAR’s, timely ordering of medications, the integrity and orderliness of the medication cart/room, and the appropriate disposition of outdated medications as per policy. 2 Maintain documentation as appropriate, including, but not limited to, accurate medical tacking sheets, medical files, medical records, appointment schedules. Reviews medical documentation and ensures that said documentation is complete and current, including MARS, obtaining updated prescriptions, diet orders, physicals, and other required medical documentation from other team members/families. 3 Reviews all training sheets to ensure all staff have been trained on all medical interventions including, but not limited to Plans of Nursing Service (PONS), Bowel Tracking Forms, etc. Notifies RN if any staff have not been trained. 4 Reviews all medical tracking sheets and MARS daily to ensure staff have been documenting appropriately and accurately and notifies RN and staff if anything is missing. 5 Provides feedback, information, and shares concerns regarding supported individuals with the Assistant Director of Nursing or designee. 6 Monitors vital signs and response to medication and treatments per PONS/MAR/prescriptions, report any abnormal findings to RN. 7 Provides individual care as directed based on each individual’s PONS. 8 Participates as directed in identification and prevention of abuse of supported individuals and identification of or provision of care in emergency situations. 9 Responds to crisis situations and medical emergencies, including accidents, by taking quick and decisive action in absence of nurse. 10 Provides treatments including, but not limited to monitoring bowel and bladder elimination as needed, tube feeding and wound/stoma care. 11 Completes and maintains training and certification in any health maintenance or medical care skills required by the assigned program. This includes but is not limited to: AMAP, G-tube, Ostomy care, Diabetes (insulin) care, Wound care, CPR, SCIP-R, etc.) 12 Acts as a role model for staff in the facilitation of medical appointments and the implementation of health and medical treatments. 13 Ensures that all individuals’ medical records comply with all applicable state regulations and requirements of The Arc Rockland. 14 Ensures that all supplies (medication, nursing forms, etc.) are available and current. 15 Supports the interdisciplinary approach to individual care as well as team process and participate as directed by Assistant Director of Nursing. ADDITIONAL RESPONSIBILITIES 1 Supports therapeutic and behavioral plans in partnership with clinical staff to enhance the quality of life for supported individuals. 2 Attends monthly staff meetings and other related meetings. 3 Summons police, fire, ambulance, or other emergency personnel if situation warrants. 4 Attends and participates in required staff training programs. 5 Supervises other health-related activities to maintain program quality. 7 Assists in all health-related care provided to the individuals at the work site. 8 Performs other related duties as requested by the Assistant Director of Nursing or Designee.
