Job DescriptionDescription: The Nurse Practitioner (NP) provides comprehensive primary and preventive healthcare services to patients at Damian Family Health Care Centers. The NP is expected to provide advanced health care to people of all ages; prescribing treatment modalities, establishing plans of care and promoting the continuum of care with other health care providers. Essential Functions • Conduct comprehensive patient assessments, including medical histories, physical exams, and diagnostic evaluations., • Diagnose acute and chronic medical conditions within scope of practice, • Order, interpret, and evaluate laboratory tests, imaging, and diagnostic studies, • Develop and implement individualized, evidence-based treatment plans, • Prescribe medications and therapeutic interventions in accordance with NYS law and DEA requirements, • Manage chronic disease conditions (e.g., diabetes, hypertension, asthma), • Provide preventive care, immunizations, screenings, and health risk counseling, • Educate patients and families on disease prevention, treatment adherence, wellness, and lifestyle modification, • Promote health literacy and culturally appropriate care, • Coordinate referrals and collaborate with specialists, behavioral health providers, and social services, • Support care transitions, follow-up planning, and continuity of care, • Maintain accurate, timely, and complete electronic medical records, • Ensure compliance with HIPAA, OSHA, infection control, and NYS regulatory standards, • Participate in quality improvement initiatives, peer review, and population health activities, • Follow clinical protocols, DFCC policies, and reporting requirements, • Work collaboratively with physicians, nurses, medical assistants, care coordinators, and interdisciplinary teams providing guidance or mentoring as appropriate, • Contribute to case conferences, care planning, and clinical meetings, • Support a respectful, professional, and patient-centered care environment, • Maintain active licensure, certifications, and required continuing education, • Attend mandatory organizational training and meetings, • Demonstrate ethical practice, sound clinical judgment, and accountability, • Support the mission, values, and community health goals of Damian Family Care Centers, • Maintain punctuality, attendance, and adherence to professional standards Non-Essential Functions • Provide cross-coverage, training, or student supervision at alternate clinical sites, • Participate in community outreach, health fairs, and patient education events, • Support grant initiatives, research, or special clinical projects, • Participate in program development or policy committees Requirements: Qualifications & Skills • Current New York State Nurse Practitioner license and registration, • National NP certification in appropriate specialty (e.g., Family, Adult, Pediatric, Women’s Health), • Current DEA registration (or obtain within 30 days of hire), • Master’s or Doctoral Degree in Nursing from an accredited program, • Current BLS certification (ACLS preferred if applicable), • Minimum one (1) year of clinical experience preferred, • Proficiency in electronic medical records (EMR/EHR), • Strong clinical judgment, communication, and organizational skills, • Commitment to health equity, patient advocacy, and culturally competent care Physical & Work Requirements Reasonable accommodations may be made to enable qualified individuals with disabilities to perform essential function • Ability to stand, walk, and move throughout a clinical environment for extended periods, • Frequent direct patient contact across pediatric, adult, and geriatric populations, • Exposure to communicable diseases, bodily fluids, and clinical equipment; PPE required, • Manual dexterity to perform examinations and minor procedures, • Ability to lift or move up to approximately 25 pounds, • Visual and auditory acuity sufficient for patient assessment, documentation, and communication, • Fast-paced environment requiring multitasking, adaptability, and time management, • Ability to communicate effectively in person, by phone, and electronically
