We are seeking an experienced and motivated Registered Nurse to serve as our Director of Patient Services (DPS) and lead our clinical operations. Position Summary The Director of Patient Services (DPS) is responsible for overseeing all clinical and patient care services, ensuring compliance with New York State regulations, and maintaining high standards of quality and safety. This role requires strong leadership, organizational, and clinical management skills. Key Responsibilities • Serve as the agency’s designated Director of Patient Services in accordance with NYS Department of Health regulations, • Oversee and manage all patient care services and clinical staff, • Ensure compliance with state, federal, and agency policies and procedures, • Supervise, train, and evaluate nursing and clinical personnel, • Conduct patient assessments, care plan reviews, and case conferences, • Monitor quality assurance and performance improvement programs, • Ensure proper documentation and regulatory reporting, • Collaborate with management, caregivers, and referral sources, • Participate in audits, surveys, and inspections as required, • Support agency growth and patient satisfaction initiatives Qualifications • Current, active Registered Nurse (RN) license in New York State (Required), • Eligible to serve as DPS under NYS regulations (Required), • Minimum of [2–3+] years of nursing experience, preferably in home care or community health, • Prior supervisory or management experience preferred, • Strong knowledge of NYS home care regulations, • Excellent communication and leadership skills, • Strong organizational and time-management abilities, • Proficiency with electronic medical records (EMR) and EVV systems preferred Compensation & Benefits • Competitive salary based on experience, • Hourly and per visit rates, • 401k, • Paid time off, • Professional development opportunities, • Supportive and collaborative work environment

You are not expected to cover all boroughs—we are building a dedicated 12‑RN team for this program and will assign you to cases near your home area whenever possible.​ You will combine flexible field work and virtual assessments to evaluate members’ daily living abilities and help determine the right level of home care services.
