ph3General Purpose /h3 pThe primary purpose of your job position is to provide each of your assigned residents with routine daily nursing care and services in accordance with the resident’s assessment and care plan, and as may be directed by your supervisors. /p h3Essential Duties /h3 ul liUse the wristband or photo card file to identify residents before administering treatments, serving meals, etc., as necessary. /li liUse only authorized abbreviations established by this facility when recording information. /li liReport all changes in the resident’s condition to the Nurse Supervisor/Charge Nurse as soon as practical. /li liRecord all entries on flow sheets, notes, charts, etc., in an informative, descriptive manner. /li liReport all accidents and incidents you observe on the shift that they occur. /li liAgree not to disclose resident’s protected health information and promptly report suspected or known violations of such disclosure to the Administrator. /li liReport any known or suspected unauthorized attempt to access facility’s information system. /li liPerform only those nursing care procedures that you have been trained to do. /li liEnsure that the resident’s room is ready for receiving the resident (i.e., bed made, name tags up, admission kit available, etc.). /li liGreet residents and escort them to their room. /li liIntroduce resident to his/her roommate, if any, and other residents and personnel as appropriate. /li liMake resident comfortable. /li liInventory and mark the resident’s personal possessions as instructed. /li liStore resident’s clothing. /li liAssist residents with packing their personal possessions when they are being transferred to a new room, or when being discharged. /li liTransport residents to new rooms or to the receiving area. /li liAssist with loading/unloading residents’ to/from vehicles as necessary. /li liCreate and maintain an atmosphere of warmth, personal interest and positive emphasis, as well as a calm environment throughout the unit and shift. /li liMeet with your shift’s nursing personnel, on a regularly scheduled basis, to assist in identifying and correcting problem areas, and/or the improvement of services. /li liReport all complaints and grievances made by the resident. /li liParticipate in and receive the nursing report as instructed. /li liFollow established policies concerning exposure to blood/body fluids. /li liMake beds as instructed. /li liPut extra covers on beds as requested. /li liMeasure and record temperatures, pulses, and respirations (TPRs) of residents, as instructed. Weigh and measure residents as instructed. /li liEnsure that residents who are unable to call for help are checked frequently. /li liAnswer resident calls promptly. /li liCheck residents routinely to ensure that their personal care needs are being met. /li liAssist residents with identifying food arrangements (i.e., informing resident with sight problem of foods that are on his/her tray, where it is located, if it is hot/cold, etc.). /li liRecord the resident’s food/fluid intake. Report changes in the resident’s eating habits to the Nurse Supervisor/Charge Nurse. /li liKeep residents’ water pitchers clean and filled with fresh water (on each shift), and within easy reach of the resident. /li liPerform after meal care (i.e., remove trays, clean resident’s hands, face, clothing, etc.). /li liParticipate in and receive the nursing report upon reporting for duty. /li liPerform all assigned tasks in accordance with our established policies and procedures, and as instructed by your supervisors. /li liFollow work assignments, and/or work schedules in completing and performing your assigned tasks. /li liCooperate with inter-departmental personnel, as well as other facility personnel to ensure that nursing services can be adequately maintained to meet the needs of the residents. /li liNotify the facility when you will be late or absent from work. /li liReport occupational exposures to blood, body fluids, infectious materials, and hazardous chemicals in accordance with the facility’s policies and procedures governing accidents and incidents. /li liAssist residents with daily functions (dental and mouth care, bath functions, combing of hair, dressing and undressing as necessary). /li liKeep residents dry (change gown, clothing and linens, when it becomes wet or soiled). /li liAssist residents in preparing for medical tests (i.e., lab work, x‑ray, therapy, dental, etc.). /li liAssist with lifting, turning, moving, positioning, and transporting residents into and out of beds, chairs, bathtubs, wheelchairs, lifts, etc. /li liPerform restorative and rehabilitative procedures as instructed. /li liAssist in preparing the resident for a physical examination. /li liCheck each resident routinely to ensure that his/her personal care needs are being met in accordance with his/her wishes. /li liObserve and report the presence of pressure areas and skin breakdowns to prevent decubitus ulcers (bedsores). /li liReport injuries of an unknown source, including skin tears. /li liObserve disoriented and comatose residents. Record and report data as instructed. /li liPerform special treatments as instructed. /li liImmediately notify the Nurse Supervisor/Charge Nurse of any resident leaving/missing from the facility. /li liUse only the equipment you have been trained to use. /li liReport defective equipment to the Nurse Supervisor/Charge Nurse. /li liMaintain the confidentiality of all resident care information including protected health information. Report known or suspected incidents of unauthorized disclosure of such information. /li liReport all allegations of resident abuse and/or misappropriation of resident property. /li liHonor the resident’s refusal of treatment request. Report such requests to your supervisor. /li /ul h3Supervisory Requirements /h3 pThis position has no supervisor responsibilities. /p h3Qualification /h3 h3Education and/or Experience /h3 pMust possess, as a minimum, an 10th grade education. /p h3Language Skills /h3 pAbility to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations. /p pAbility to write reports, business correspondence, and procedure manuals. /p pAbility to effectively present information and respond to questions from managers and employees. /p h3Mathematical Skills /h3 pAbility to apply concepts such as fractions, percentages, ratios and proportions to practical situations. /p h3Reasoning Ability /h3 pAbility to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists. /p pAbility to interpret a variety of instructions furnished in written, oral, diagram, or schedule form. /p h3Certificates, Licenses, Registrations /h3 pMust be a licensed Certified Nursing Assistant in accordance with laws of the state. /p h3CPR Preferred /h3 pCPR certification preferred. /p h3Physical Demands /h3 pThe physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job.
