We are looking for a reliable and experienced Home Care Assistant/Reablement Support Worker to provide two care calls per day, one in the morning and one in the afternoon, each lasting 1 hour and 30 minutes, to support an elderly female service user in the CM20 postcode area following hospital discharge. The role includes support with safe transfers, mobility, toileting, personal care, meal and drink preparation, medication prompting, nutrition support, falls prevention, light domestic tasks, and clear daily record keeping. The aim is to promote the service user’s safety, dignity, comfort, confidence, and independence at home. Applicants must have a caring approach, good communication skills, and experience in safe moving and handling.
