Community Habilitation Specialist Job Description:

As a Community Habilitation Specialist, you will support people in recreational, social, and skill-building activities within their homes to achieve their goals while encouraging independence and personal choice.
SKIP of New York is a not for profit organization transforming lives and providing services for medically fragile kids and families since 1983.

Community Habilitation Specialist – Duties, and Responsibilities:
  • Work 1:1 with a person with developmental disabilities to encourage independence as appropriate throughout daily routines and provide opportunities for people to participate in making choices as appropriate.
  • Engage participation in recreation, social, and skill-building activities as appropriate.
  • Complete documentation electronically as required.


    Community Habilitation Specialist Qualifications/Skills:
  • HS/GED
  • Prior experience working with people with developmental and intellectual disabilities is preferred.
  • Effective oral and written skills in English.
  • Must work positively within a team environment and be able to work independently.
