The Day Habilitation Specialist is responsible for providing support services to individuals in the Day Habilitation Program to reach their individualized goals in activities, community integration and skill building. The Day Habilitation Specialist is responsible for working positively with consumers in achieving their goals and documenting their progress in accordance with regulations. The Day Habilitation Specialist is responsible for developing a comprehensive understanding of each consumer and the appropriate supports to be offered. This position requires substantial interaction within the community and requires professionalism and courtesy to be applied to all situations. This position reports directly to the Manager of Day Habilitation Services. RESPONSIBILITIES: - Assist individuals in meeting their goals as identified in their Day Habilitation plan and maintain daily documentation. - Offer appropriate encouragement and support to individuals in achieving their outcomes while ensuring their health and safety as outlined in their clinical assessment, ISP, Day Hab plans, behavior plans, etc. - Provide hands-on training to assist individuals to develop skills, partake in chosen activities, and participate in community activities. - Responsibilities include engaging in meaningful activities on-site, as well as travel with the individuals to off-site and community locations. - Participate in interdisciplinary team meetings to provide input in planning and advocacy. - Meet with supervisor on a regular basis to review individual issues and recommend program improvements for quality services to the individuals. - To attend staff trainings as required. - To report all incidents to management as per OPWDD regulations. Other responsibilities as assigned by supervisor. - Ensure that proper staff ratio is maintained at all times which may necessitate a change in your work schedule, and or location. Minimum Experience: - One year experience in an OPWDD setting or similar, good interpersonal skills. - Ability to provide training in areas as travel training, money management and independent living skills, etc. Preferred Education: Associate's degree Preferred Experience: - Two years working in a Human Services field, with at least one of these in an OPWDD setting. - Experience providing training in areas including travel training, money management and ADLs, etc. Valid First Aid, CPR, SCIP, AMAP Certifications.
The Direct Support Professional is responsible for assisting individuals with developmental disabilities and/or other impairments to live as independently as possible within the community. The position is required to maintain a professional and positive attitude while encouraging people we support to pursue their personal interests at home and in the community. Due to the Direct Support Professional’s involvement in all aspects of people we support daily living, this role requires confidentiality of the individuals’ personal information in accordance with HIPAA regulations; strong communication skills to work effectively with the people we support, families, co-workers and within the community; ability to problem-solve and evidence good judgment in implementing plans of service. This position requires current IRI driving authorization, AMAP, SCIP-R and Standard First Aid/CPR certification. RESPONSIBILITIES: ❖ Be familiar with individual resident’s ISP, Res Hab Plan, Goal Plans, Behavior Modification Plan, etc. ❖ Assist individuals with ADL and other independent living skills in accordance with plan documents. This assistance may take the form of full support, partial support or verbal prompting, as well as behavioral interventions as indicated in the behavior management plan. These tasks may include grooming, housekeeping, budgeting, shopping, etc. according to support needs of the individual. ❖ Promote and advocate for individualized services and implementation according to the plan. Document service provision as indicated in plans and regulations including but not limited to res hab goals, behavior data, sleep charts, logbooks, etc. ❖ Ensure protective oversight is provided to the people we support in accordance with their Plan of Protective Oversight. ❖ Accompany the people we support on community outings. ❖ Communicate with other program staff via the communications logbook and attendance at staff meetings. ❖ Work collaboratively with other members of staff in maintaining a positive, encouraging environment to support the people we support. Participate in interdisciplinary team meetings to assist in the development of a person-centered plan. ❖ Adhere to all federal, state and city regulations regarding service provision and documentation. ❖ Report all incidents as per IRI policies and procedures. ❖ Conduct fire drills and document results in accordance with supervisor’s instructions. ❖ Attend staff meetings and trainings on an on-going basis to increase knowledge for the individuals we support and best practices. ❖ Maintain certifications and driving authorization. ❖ Additional duties as assigned by supervisor. ❖ Often the requirement to work overtime is mandatory to ensure that proper personnel coverage is maintained at IRI facilities. ❖ Ensure that proper staff ratio is maintained at all times which may necessitate a change in your work schedule, and or location. QUALIFICATIONS: Minimum Education: High School Diploma or GED Minimum Experience: - Good interpersonal skills and commitment to assisting individuals with disabilities. - At least 6 months experience working in an environment with people. - Ability to become certified in AMAP, SCIP-R, Standard First Aid /CPR. Valid NYS driver’s license with less than 6 points. The Medical Care Worker is responsible for the day-to-day management of medical/clinical services provided to the residents within residences operated by Independence Residences, Inc. The MCW is also responsible for the provision of direct support services to the residents as needed. RESPONSIBILITIES: ❖ Attend monthly staff meetings, IDT and MCW Meetings as required. Report upcoming MAR’s at least 3 days and print. ❖ Follow policies and procedures set forth in the residential policy and procedure manual. ❖ Maintain a current and up to date medical appointment calendar in the residential facility. ❖ Ensure all upcoming and scheduled appointments are logged immediately on the appointment calendar. And report all psychotropic changes to ABS. ❖ Schedule and track all medical appointments including follow-ups as needed or recommended within designated time frame. Keep van clean after use and report issues immediately to supervisor. ❖ Accompany individual(s) to and from appointments unless otherwise instructed. ❖ Advocate and act as a liaison while in the community. ❖ Prepare clinic forms and attach all medical information prior to the appointment (e.g. appointment card, lab results, and copy of previous clinic form, Medicaid card etc.). Information should be prepared minimally one day in advance. ❖ Review clinic forms prior to leaving the clinic/service provider to ensure the doctor’s recommendation/order is clearly written, and all prescriptions include start and stop dates. ❖ Communicate medical findings in an informative, professional manner (e.g. report results to Program Management, and RN). And submit Dr orders to RN for review, then PCP then take to pharmacy ❖ Submit clinic visit forms copy to day programs. ❖ Communicate all pertinent medical documentation in the communication log.
