The responsibilities include: Serving as a member of the location's team committed to helping young people build on existing assets to make positive choices about their health and well-being, academic achievement and interaction with others. Coordinating the implementation of Individual Service Plans (ISP) for each youth in accordance with agency ISP planning policies and procedures. Establishing and monitoring the methods used to teach the youth new skills as per individual ISP’s and works with program staff to provide services in accordance with established methods. Coordinating and linking each youth to off-site support services as defined in ISP. Planning the transportation for the youth’s participation in off-site services. Completing all case management documentation and reports and maintaining individual youth files. Reviewing medication logs daily. Conducting weekly audit of medication logs to ensure compliance with medication administration policy. Implementing behavior modification plans established by the team to address discipline issues and the consumer’s lack of adherence to program rules, as needed. Essential Duties and Responsibilities: · Provides input and advice regarding the programs’ primary and preventative health care components. · Provides individual consultation to consumers on heath-related matters and to program staff regarding residents’ specific medical problems, concerns or needs. · Assesses consumers and makes referrals as necessary. · Conducts a weekly review of medication administration records to ensure accuracy and compliance; also conducts a monthly review of a sample of resident health care records to ensure that appropriate follow-up care is received. · Participates in monthly clinical case conference meetings. · Creates an annual calendar of training workshops for both residents and staff and conducts the workshops. Training topics should include, but are not limited to, the following topics: Vital Signs; Diabetes Education; General Nutrition; Diet, Exercise and Weight Loss; Workplace Safety; and General Health and Wellness. · Participates in quarterly CQI Committee meetings. · Cooperates with the Division of Developmental Disabilities and Office of Licensing during all investigations. · Other duties as assigned. NJCDC delivers programs that support youth, families, and communities through education, prevention, and social services. Position Overview: The Licensed Clinician will provide clinical services to youth and families both in-person and through telehealth. This role includes screening, assessing, and providing brief individualized clinical interventions, facilitating treatment referrals, and collaborating with school personnel and community partners. The clinician may also provide training and supervision to other clinicians who do not possess independent licensure. Key Responsibilities: Provide clinical services to students and families both in-person and via telehealth. Screen, assess, and provide brief interventions to youth and families; stabilize students until long-term resources are identified. Facilitate treatment referrals and connect students/families to appropriate community supports. Collaborate with school personnel and community partners to provide comprehensive services. Provide school consultation services to staff and families as needed. Maintain accurate and timely clinical records and documentation.
