RehabCare is seeking a dedicated and compassionate Occupational Therapist to join our team in Bronx, NY. In this role, you will be instrumental in helping patients regain independence and improve their quality of life. Key Responsibilities: • Conduct comprehensive assessments of patients' physical, cognitive, psychosocial, and environmental factors impacting their daily living., • Develop and implement individualized treatment plans focused on therapeutic activities, adaptive equipment, and environmental modifications., • Provide direct patient care, including training in activities of daily living (ADLs), instrumental activities of daily living (IADLs), and work-related tasks., • Educate patients, families, and caregivers on compensatory strategies, energy conservation techniques, and home exercise programs., • Collaborate effectively with physicians, physical therapists, speech-language pathologists, and other healthcare professionals to ensure coordinated care., • Maintain accurate and timely documentation of patient evaluations, treatment plans, progress notes, and discharge summaries., • Adhere to all professional standards, ethical guidelines, and regulatory requirements. Qualifications: • Valid state licensure as an Occupational Therapist., • Master's or Doctorate degree in Occupational Therapy from an accredited program., • Strong clinical assessment and treatment skills., • Excellent communication, interpersonal, and organizational abilities., • Ability to work independently and as part of a multidisciplinary team., • Commitment to patient-centered care and continuous professional development.
