JOB PURPOSE: • To deliver a high-quality service for semi-independent accommodation and support to young people aged 16 plus who are preparing to leave the care of the local authorities. • To support young people to develop independent living skills through keywork, goal setting and outcome focused activities. • Working with young people with varying emotional and behavioural needs. • Responsible for promoting a positive, safe and homely environment for all young people within the care of Eleven D’s. • To act as a role model to all young people placed at Eleven D’s. • Ensure all records and daily logs are completed and maintained fully up to date. • To work in accordance with Eleven D’s Mission statement, values, policies and procedures. MAIN DUTIES AND RESPONSIBILITIES • Managing young people living in semi-independent accommodation. • Responsible for structuring key work sessions consistent with an independent placement agreement (IPA) and ant care / support plans. • Develop a structured programme of key work sessions based upon the young person’s individual support needs and focused on developing their self-confidence, resilience and self-esteem and independence. • Complete full induction of new residents and support the young person in settling within the community and accessing local resources. • Ensure the health, safety and wellbeing of each young person and that safeguarding procedures are followed throughout their care at Eleven D’s. • Maintain the confidentiality of young people and report any concerns regarding the young person’s welfare, missing episodes etc to your line manager once the Emergency Duty Team (EDT) and police have been contacted if needs be. • Assist the young person to engage with everyday living and develop skills where necessary by supporting their identified needs. • Provide support to young people with budgeting money, grocery shopping and administer a weekly subsistence allowance. 1 • Encourage and support young people in accessing education, employment or training as well as supporting them with the application process. • Encourage the young person to engage and take part in positive activities within the community, building on relationships and developing social networks. • Maintain accurate records, files, log keeping and all other required paperwork and administrative requirements. • Maintain petty cash and receipts for each expenditure. • To maintain the cleanliness and hygiene of the Home, reporting all repair issues and ensuring the environment is always welcoming for the young people. • You will be responsible to carry out daily / weekly Health and Safety checks within the building. You must record and report any repair work or damage to the property to ensure it is kept to a high standard. • Promote safe, consistent and understandable boundaries with young people in conjunction with a 'Young Person’s Agreement’, Support Plan and House Rules. • Prepare detailed high-quality monthly progress reports regarding each young person’s progress which is submitted to the placing authority within deadlines. • Work in partnership with social workers other agencies and to encourage and support the young people to attend meetings with other professionals. • Attend admission and planning meetings, and attend Looked after Children Reviews ensuring all relevant paperwork is completed. • Mandatory attendance and participation in team meetings and training sessions as and when required. • Mandatory attendance and participation in structured supervision sessions at least once a month.

All our crews/drivers are responsible for the safe transportation of informal and patients detained under the Mental Health Act, diagnosed with mental health conditions. It is a demanding yet very rewarding service that we provide. We are looking for bright and motivated individuals with an enhanced DBS and Full UK Driving Licence to join our dedicated team. We provide support to both independent and NHS hospitals and services to pick up and convey patients who may be acutely unwell, from hospitals including local A&Es, Health based Place of safety, home addresses, Courts, and police stations. We are 24 hours a day, 7 days a week service and can potentially offer hours around your individual commitments. We do have full-time staff members that regularly work over 37.5 hours a week, but you are able to work as little or as often as suits you, dependant on the amount of work available. Would you like to be part of a very dedicated team around a friendly 17 year old disabled teenager who lives at home with his family and dog. He is a friendly, non verbal, very autistic boy with severe epilepsy and additional health needs. He needs help with all elements of his daily life. About you: This job will suit someone who has experience supporting a young person with disability and autism or has worked with disabled people before. You need to be open to the care elements in this job or have already some care experience but this is not just a care job but also very much about support and we would like to find someone who can be active, fun and shows initiative and really needs to be quite fit to support a teenager with a movement disorder. This could potentially suit a student of a related field as a part time position (28+ hours) or as full time (35+ hours). It would suit someone who is creative, very open minded and enthusiastic but has also worked in the field before. This is very different from working in a care home. You need to be able to swim (absolute requirement). You will be dedicated and committed. Need to be fun and responsible. A non-smoker / non vaper (requirement). Not afraid to learn care elements like catheterization, peristeen and dealing with PEG (ideally you have experience already - otherwise we train you). Strong interest in disability and autism (absolute requirement). You would be willing to commit for a minimum of 12 months. We hope to find someone who is open to work some weekends and some bank holidays too (shared with the team). You should be happy to support the young person with his family for occasional weekends away and on his yearly summer holiday (usually for 1 week). You would also be open to take on a weekly waking night support shift and occasionally more. What’s great about this job: Fantastic training opportunities, with a thorough handover, from long-standing members of staff who know the young person and his family very well. You will be provided with regular training on his health and care needs which include seizure management, catheterisation and peg management and behavioural support. About the friendly autistic teen: He lives with his loving and supportive family, who recognise that his team of support workers & carers are instrumental in his overall development and well being. When he is well he likes physical activities — like going to the pool, walks in parks, disability bike rides and going to cafes for hot chocolates. He needs support with all of these activities as he also has a movement disorder. He also likes to chill with his sister and watch movies and join in family meals. Ideally you are someone who has initiative and can also come up with suitable activities. Unfortunately he often also needs a lot of care as he can have seizures which lead to him needing to spend time in hospital. He has also other health needs. About the job: The training is provided by the experienced team members as well as his ABA therapist. This might be a good position for someone who has a keen interest in disability, psychology, nursing and the position could suit a part time student of a relevant field.
