Position Overview: We are seeking a compassionate, experienced and organised Care Coordinator to join our dynamic team. In this role, you will be responsible for coordinating carer rotas, facilitating communication between clients, families, multidisciplinary teams, and carers to ensure that our clients receive the highest quality of care.
