Part-Time Experienced Care Coordinator Required

We are seeking an organised, proactive, and experienced Part-Time Care Coordinator to join Feldon Care. The successful candidate will play a key role in coordinating care services, supporting care staff, and ensuring the delivery of high-quality, person-centred care. Key Responsibilities: • Coordinate and schedule care visits efficiently, • Manage staff rotas and ensuring adequate care coverage, • Support care staff to deliver safe, effective, and person-centered care, • Liaise with clients, families, healthcare professionals, and care staff, • Conduct care reviews, assessments, and care plan updates, • Review and aduit care plans and care documentation, • Monitor service quality and support compliance with safeguarding requirements, Feldon Care policies and CQC standards, • Monitor the quality of care delivery and support continuous improvement, • Assist with spot checks, competency assessments and quality assurance activities, • Assist with recruitment, onboarding, and staff support, • Maintain accurate records and care documentation, • Respond to day-to-day operational and service delivery needs Requirements: • Previous experience as a Care Coordinator within domiciliary care or a similar health and social care setting, • Strong organisational and time-management skills, • Excellent communication and interpersonal skills, • Good understanding of care planning, safeguarding, and compliance requirements, • Ability to work independently and manage multiple priorities, • Competent in using Microsoft Office and care management systems, • Up-to-date Mandatory Training certificates, • Right to work in the UK, • Enhanced DBS (or willingness to obtain one), • Full UK driving licence and access to a vehicle desirable but not essential What We Are Looking For: We are looking for a motivated, professional, and reliable individual who is passionate about delivering high-quality care services.
