Job Description SUMMARY Family Service League is seeking a full-time Counselor for our Family Recovery Center (FRC) in Mastic. The Family Recovery Center provides individual and group therapy to children, families, and adults directly impacted by alcohol and drugs. The Counselor will consult and advocate with outside agencies and other supports, as needed, and facilitate referrals and follow-up on those services. RESPONSIBILITIES • The Counselor will provide the most appropriate and highest level of service to clients., • Manage a caseload of clients and provide case management services; possibly primarily mandated by probation., • Complete intake assessments., • Conduct individual and group therapy sessions., • Make accurate diagnosis and confer with supervisor when needed., • The Counselor will provide each client with a realistic crisis plan., • Work with clients to develop and reach treatment goals and change goals as needed., • Efficiently access other necessary services for client., • Complete progress notes, treatment plans and other OASAS required documentation in a timely manner., • Conduct and record all results of laboratory screenings., • Maintain contact with client legal mandates and notify of significant issues., • Facilitate groups for clients depending upon client need., • The Counselor will register appropriate clients in impaired driver system., • Correspond with collateral family members and agencies in a timely and professional manner., • Acquire knowledge of modalities relevant to client population., • Maintain reasonable kept appointment rate., • Contact clients by phone and letter regarding missed appointments., • Obtain all necessary signatures and documentation related to client care., • Maintain up-to-date progress notes, treatment plans, and Outpatient Treatment Reviews (OTRs)., • Terminate cases when appropriate and using the appropriate procedures., • Inform office staff and ensure that clients are informed of schedule changes., • Adhere to OASAS regulations., • Meet regularly with supervisor to review cases., • All other duties as assigned. QUALIFICATIONS High school diploma or equivalent required. CASAC/CASAC-T or related education required. Minimum of 2 years of experience with substance use disorders required; experience with co-occurring disorders preferred. Experience working with adolescents and teens may be required. Excellent interpersonal, organizational, problem solving, and verbal and written communication skills required. Ability to work as part of a multi-disciplinary team and the ability to work closely with referral sources required. Proficient computer skills, including Microsoft Office required; Electronic Health Records preferred. Bilingual in Spanish preferred. The Social Worker works as part of an interdisciplinary team to provide therapeutic interventions and Care Management to adolescents and adults who have behavioral health needs and who have not been successfully engaged in services during or after critical times in transition. CTI promotes community integration, self-advocacy, and access to ongoing support by helping individuals develop and utilize strong ties to their professional and non-professional support systems during and after transition periods. This is a community-based position with frequent field work, beginning with client engagement in hospital settings and providing support for approximately 9 months following discharge. RESPONSIBILITIES • The Social Worker will assess needs, coordinate services, and provide crisis intervention services to adolescents and adults with a full range of DSM diagnoses., • Educate individuals about mental health, treatment, and recovery., • Teach skills for coping with specific symptoms and stress management, including development of a crisis management plan, developing a relapse prevention plan, including identification/recognition of early warning signs and rapid intervention strategies., • Collaborate on referrals, resources, and interventions within a multidisciplinary team that includes Master's level clinicians, Care Managers, Peer Specialists, and nurses., • Facilitate linkages and coordinate care planning with other providers of services/resources to ensure goal directed, collaborative care, including care transitions., • The Social Worker will coordinate with collateral contacts, including family and other providers., • Assess for Social Determinants of Health and facilitate linkages to community providers and supports., • The Social Worker will establish and maintain positive working relationships with others, both internally and externally., • Complete all clinical documentation in accordance with NYS Office of Mental Health in an electronic medical record., • Assess situations to determine importance, urgency and risks, and make clear decisions which are timely and in the best interests of the organization., • The Social Worker will set priorities, develop a work schedule, and monitor progress towards goals, and track details, data, information, and activities., • Provide support to clients in the community with in-person, field-based interaction, as well as in-office settings., • The Social Worker will provide services to clients beginning in hospital settings and spanning approximately 9 months following discharge., • Coordinate and connect to community resources., • Attend routine supervision/team meetings., • Complete training and maintain proficiency with de-escalation skills., • Abide by FSL's Code of Ethics., • All other duties as assigned.
