Options for Community Living, Inc. is looking for a Medication Management Specialist to join our team! When determining a salary offer, several factors may be considered as applicable (e.g., years of relevant experience, education level, language skillset, credentials, professional licensure, budget, and internal equity). Schedule: Monday, Wednesday, Friday: 9:00 AM - 5:00 PM and Tuesday, Thursday: 12:00 PM - 8:00 PM (35 hours/week) Location: In person; Ronkonkoma site with field visits required. Pay Type: Non-exempt Responsibilities: The Medication Management Specialist provides various support services to residents with psychiatric disabilities in the Apartment Treatment Program. Reports to the Program Supervisor. • Provide oversight and monitoring of client medications at the homes of identified clients who are in need of this service., • Link residents with appropriate resources., • Engage in ongoing assessment of client needs to determine frequency of medication monitoring., • Provide education and resource information regarding medication and medical and mental health disorders., • Prepare clients to monitor and take their medications independently., • Complete timely submission of medication management progress notes, assessments and other relevant documentation in compliance with department and agency standards., • Conduct overall assessment of client welfare and appropriate intervention and follow-up as needed., • Participate in on-going communication with medication prescribers, service providers, Case managers, and any other collateral contacts as needed., • Any other relevant tasks as assigned by management., • Provide training on medication management and supervision for Licensed Program staff.
