Job Description JOB TITLE: RN Case Manager / Nursing Supervisor • This hybrid role requires a mix of case management, recertifications, starts of care (SOC), resumptions of care (ROC), other assessments and services, occasional Skilled Nursing visits, as well as new hire orientations, trainings, in-services, and supervisory reviews., • At least 1 year of Case Management experience and home health agency (or equivalent) experience are required., • Must have experience with OASIS and NON-OASIS assessments., • No On-Call requirements / No Overnight shifts required., • HCN will expand this role into Full-Time (choice of hourly or salary) for the right candidate., • Weekend, Overtime, Respite & PRN services are offered when available but are not required., • Includes Summit, Medina, Cuyahoga, Tuscarawas, and surrounding Counties, primarily aligned along I-77, • Qualified applicants may reside in or near any of the primary service area counties so long as travel requirements are fulfilled to all stated counties., • Primary Service Area Counties include: Summit, Medina, Cuyahoga, Tuscarawas, primarily aligned along I-77., • Flexible and customizable. To support our growth and the increasing volume of client care throughout Ohio, we are seeking a highly skilled, experienced, accountable, and self-motivated home health RN Case Manager (RNCM) / Nurse Supervisor to join our agency and serve existing and returning clients throughout 4 counties in NorthEast Ohio. • This position requires travel to multiple counties but offers a consistent schedule to fit your personal / family commitments (no overnight shifts or overnight travel). Responsibilities/Requirements of an RN Case Manager / Clinical Nurse Supervisor include: • RN performs client assessments while directing, planning, and initiating appropriate actions in home care situations., • RN manages/supervises a team of RN, LPN, Home Health Aide (HHA), and Personal Care Aides., • RN to Document/Communicate/Coordinate patient status/change in condition to care team., • RN to perform job duties while promoting personal safety and a safe environment for clients and coworkers., • Willing to travel surrounding counties., • Graduate of an accredited School of Nursing., • Minimum of 1 year experience as a Registered Nurse, with an active RN License / ability to practice in the State of Ohio., • CPR and First Aide Certifications required (or can be obtained prior to hire date through our agency or accredited third parties)., • 1 year Experience in case management and home health required, with 2+ years of home health experience highly preferred., • Experience in a supervisory role, wound care, lab draws, and OASIS assessments is a plus., • Willingness to travel throughout all stated counties in this service area to care for clients is required., • Strong background in nursing, patient care, and confidentiality., • Strong attendance and dependability track record with previous employers., • Desirable character traits: reliable, honest beyond reproach, professional, compassionate, detail-oriented (in care + documentation), accountable, friendly and accommodating, respectful, and positive., • Familiarity with HIPAA regulations and medical terminology., • Current Driver's License and reliable transportation.
