The Assistant Case Manager assists with coordination and provision of support services to clients of the housing program. Implement contract regulations and assist with preparation of necessary reports to funding sources. What You Will Do: • Support individuals to reach their personal goals, from daily tasks to personal achievements., • Provide transportation to fun activities, medical appointments, and more, ensuring individuals stay connected with their community., • Support behavioral modifications and communication improvements, helping individuals thrive in every aspect of their lives., • Work with the individuals and other members of the team to develop service plans that aid the person in achieving their life goals., • Support and promote the individual’s health and safety., • Maintain accurate and thorough documentation of service activities, including daily progress notes, interactions, and any additional information as required by agency guidelines.
