About the Role As a Clinical Pharmacist Facilitator, you will work within primary care settings to provide expert medicines management support, education and clinical guidance to practitioners and patients. Your role combines clinical pharmacy, relationship building and quality improvement to support safer, more effective use of medicines across the region. Key Responsibilities • Deliver independent medicines reviews., • Provide unbiased clinical advice and interpret prescribing and laboratory data., • Work with general practices to support prescribing improvement and implementation of best‑practice guidelines., • Contribute to continuous quality improvement projects., • Support education sessions and develop targeted resources., • Strengthen collaborative care between primary and secondary services.About the Team / Service You will be part of the Clinical Pharmacist Facilitator Team, working closely with general practices, community pharmacy, Māori and Pacific providers, the Health Hawkes Bay PHO team, and Te Whatu Ora's pharmacy and specialist services. The role is primarily community‑based within primary care clinics, with strong support from the wider multidisciplinary network. About You You’re a confident, collaborative clinical pharmacist with strong relationship‑building and communication skills. You will have: • Current registration with the Pharmacy Council of New Zealand and a valid APC (no restrictions)., • A relevant postgraduate clinical pharmacy qualification or working toward one, or equivalent clinical experience., • Strong clinical knowledge and experience in medicines therapy assessments, medicines information provision, quality use of medicines, continuous quality improvement, and the ability to interpret and work with clinical, prescribing and laboratory data., • Excellent communication and engagement skills with prescribers, primary care teams and patients., • Ability to work autonomously and innovatively within evolving service priorities., • Full NZ drivers licence., • Commitment to Te Tiriti o Waitangi and equity‑focused practice.Desired, • Membership with PSNZ, NZHPA or CAPA., • Project management experience., • Understanding of primary care systems (e.g., Medtech, MyPractice, Indici)., • Knowledge of local health needs and national medicines strategies., • Analytical and data‑insight capability. xcskxlj Benefits We welcome applications from our diverse Māori, Pacific, disabled, and rainbow communities. #J-18808-Ljbffr
