
Job Summary
The Care Coordinator plays a vital role in ensuring that clients receive the highest quality of care and support. This position involves coordinating services, managing schedules, and facilitating communication between clients, families, and healthcare providers. The ideal candidate will possess strong organisational skills and a compassionate approach to client care.
Responsibilities
Coordinate and manage care plans for clients, ensuring all services are delivered effectively and efficiently.
Liaise with healthcare professionals, families, and clients to facilitate communication and address any concerns.
Maintain accurate records of client information, appointments, and care plans in compliance with regulatory standards.
Monitor client progress and adapt care plans as necessary to meet changing needs.
Provide support to clients and their families by answering questions and offering guidance on available resources.
Transport customers into the community, and to and from there homes.
Experience
Previous caring experience is essential, demonstrating strong skills.
Experience in a healthcare or social services environment is highly required.
Proficiency in using systems for record keeping and communication purposes.
Excellent interpersonal skills with the ability to build rapport with clients and their families.
Strong problem-solving abilities and attention to detail are crucial for success in this role. This position offers an opportunity to make a meaningful impact on the lives of individuals while working within a supportive team environment.
Must hold a full UK driving license with business insurance cover, this is essential.
