The First Contact Practitioner Physiotherapist will be responsible for:
Acting as a First Contact Practitioner for patients presenting with musculoskeletal conditions with in primary care.
Independently assessing, clinically reasoning, diagnosing and managing patients presenting with musculoskeletal conditions.
Undertaking comprehensive clinical assessment and identifying appropriate management plans for patients presenting directly to primary care.
Identifying serious pathology, red flags and conditions requiring urgent medical assessment or onward referral.
Providing evidence-based advice, treatment and rehabilitation recommendations appropriate to the patient's clinical presentation.
Supporting patients with self-management strategies and appropriate education concerning their musculoskeletal condition.
Making appropriate onward referrals and signposting patients to relevant primary, community or secondary-care services where clinically indicated.
Requesting or recommending appropriate investigations where this falls within the practitioner's professional competence, scope of practice and locally agreed clinical pathways.
Liaising and working collaboratively with GPs, practice clinicians and other members of the multidisciplinary primary-care team.
Maintaining accurate, contemporaneous and appropriate clinical records using the relevant clinical record systems.
Working within relevant professional standards, clinical governance requirements, safeguarding procedures, infection prevention and control requirements, information-governance requirements and local PCN policies.
Maintaining appropriate continuing professional development and professional registration.
Contributing to the safe, effective and efficient delivery and development of the FCP Physiotherapy Service.
