At times they may become aggressive and /or violent. While the Agency does provide specialized training for handling Individuals when they become aggressive and/or violent, the Agency cannot guarantee the safety of its employees who come in contact with Individuals. This condition exists whether the employee is directly or indirectly assigned to Individuals. Employees assume the risk of working with our Individuals. Function: Report to the Assistant Residential Supervisor and Residential Supervisor. Assist the facility nurse and psychologist with providing medical services to the Individuals:  Aid the nurse and psychologist in the coordination of medical appointments i.e. audiology, dentistry, neurology, ophthalmology, physical examinations, Laboratory work, psychiatry, physical therapy, occupational therapy, and any other clinical services as per the Individual’s needs.  Schedule and attend clinic appointments; communicate results to the nurse, psychologist and/or residence manager including but not limited to any medication changes, need for consents, need for specimens; post follow-up appointments on the clinic calendar in the facility.  Serve as the liaison between the nurse, psychologist, residence manager or assistant manager.  Coordinate scheduling of follow-up appointments and ensure follow-up visits are carried out under the direction of the nurse. Serve as an advocate for the Individual’s by obtaining the best possible care at the clinics, hospitals and doctor’s office. Essential Functions:  Prepare clinic consults and other required paper work for appointments in the absence of the nurse under the direction of the nurse.  Coordinate and attend medical appointments under the direction of the nurse and/or residence manger.  Maintain sufficient medication supply and equipment in the absence of the nurse.  Schedule medical appointments and record appointments on the clinic calendar.  Report to the facility nurse, psychologist, and the manager and /or assistant manager regarding appointments and follow-up information.  Communicate with day programs regarding Individual’s medical appointments and changes in medical condition i.e. emergency room visits, under the direction of the nurse.  Inform management if any additional supports are needed for the Individual’s medical appointments.  Accurately use Precision Care software to maintain and track Individual’s medical information. Job Description – Clinical Direct Support Professional 12/10/2020 Page 2 of 4  Perform all tasks and standards per the Direct Support Professional (See attached job description).  All other duties as deemed appropriate by the residence manager, nurse and/or the psychologist.  Check medication supply and equipment at least twice a week and report to the nurse.  Maintain and monitor bowel movements charts, menstrual cycle sheets, and other medical monitoring systems to ensure accuracy and completion, under the direction of the nurse.  Maintain and write the monthly MAR sheets in the absence of and under the direction of the nurse.  Able to lift and transfer up to 25 pounds.  May be assigned to work with Individuals in wheel chairs, walkers, and operate Hoyer lifts if needed.  Perform other tasks as required. Must successfully pass First Aid, CPR and AED  Must successfully complete AMAP training along with AMAP Certification as provided by the New York State Office for Persons with Developmental Disabilities (OPWDD) within (60) days.  SCIP training must be successfully completed within (30) days of employment.  Must attend in-house shadowing/orientation. Requirements:  Valid Driver’s License  HS Diploma/GED  Proficient reading and writing skills  Effective communication skills  Knowledge of basic medical terminology  Good organizational and time management skills  Basic computer skills  Valid AMAP certification  Good employment standing COVID-19 Protocols All staff are required to follow procedures in decreasing the spread of COVID-19. Requirements are as follows:  Personal Protective Equipment (PPE) - Wearing agency approved face coverings/masks when entering the building and working within the program at all times.  All staff are REQUIRED to participate in daily screenings prior to entering the building.  Frequently wash & disinfect hands (using soap and water or hand sanitizer that contains at least 60% alcohol).  Avoid touching your eyes, nose, and mouth with unwashed hands.  Clean AND disinfect frequently touch surfaces (work stations, items, knobs, light switches, etc.).  Social distance – remain six feet between yourself and others when it is appropriate.
