Job Description POSITION: Home Study/Post-Release Services Case Manager REPORTS TO: Case Management Supervisor, Regional Supervisor CLASSIFICATION: Full Time STATUS: Exempt SALARY: 55,000-57,000 Position Summary: The “Home Study/Post Release Services” program (“HSPRS”), operated by Lutheran Immigration and Refugee Service (“LIRS”), provides supportive services to unaccompanied immigrant children after release from ORR custody. Reporting to LIRS’s Regional Supervisor and LSSNY’s Case Management/Personnel Supervisor, the HS/PRS Case Manager will conduct home studies, case management and supportive services to unaccompanied immigrant children once they are released to their sponsors in the New York metro area. Job Description JOB DEFINITION: The Care Coordinator works with the CN Guidance Critical Time Intervention Team to deliver services within an evidence-based model that is time limited, phase-based care management approach focused on enhancing continuity of care during transitional times. Care Coordinators develop the person center plan, share knowledge of community resources, provide linkage to resources and engage individuals in skill building. EDUCATIONAL REQUIREMENTS: Bachelor's Degree or Master's Degree in one of the following fields preferred: Social Work, Psychology, Education, Rehabilitation, Occupational Therapy, Counseling, Community Mental Health, Sociology, Speech and Hearing, Physical or Recreational therapy.
