General Description Provides direct clinical care, integrated treatment, rehabilitation, and support services to ACT recipients. Meets with recipients regularly in the community. Qualifications and Competencies • Master’s Degree in Human Services; licensed/certified (SW, CASAC, OT, Psychologist, etc.)., • 3 years human services experience; 1 year specialty preferred. Duties and Responsibilities • Provide face-to-face clinical services in the community., • Support daily living and person-centered goals., • Offer counseling, housing/job assistance, and wellness supports., • Develop and modify treatment and recovery plans., • Ensure rights and compliance with ACT guidelines., • Dual Diagnosis Treatment (harm reduction, relapse prevention)., • Employment counseling and vocational goal development., • Family support and psychoeducation., • Linking to community resources and entitlements. Physical Demands & Work Conditions • Travel to sites as needed., • Transport clients in personal/agency vehicle., • Work staggered shifts 8AM–8PM with on-call rotations.
