Job DescriptionPOSITION SUMMARY: The Behavioral Health Therapist will provide effective culturally competent behavioral health assessments and therapy to individuals, families, and groups in a community-based, in-person, Primary Care setting, working in concert with an integrated primary care team and promoting integrated behavioral health. As a member of the health center’s integrated care team the Behavioral Health Therapist will fulfill the following KEY FUNCTIONS AND RESPONSIBILITIES towards the provision of patient-centered, coordinated patient care: • Maintain confidentiality of records relating to patients' treatment., • Provide mental health and substance abuse assessments, • Provide Individual, Couples, and family short-term mental health treatment for adults, children, and teens using evidence-based interventions such as CBT, MI, trauma-informed care, and/or Solution-Focused Brief Therapy, • Conduct substance abuse screenings and brief counseling or referral for addictions treatment., • Develop and implement treatment plans based on patients’ expressed goals. Complete and maintain client-related records in the EMR (ECW), daily., • Follow regulations for documentation and billing., • Collaborate and/or consult with other team members to perform clinical assessments or develop treatment plans., • Coordinate clinical approaches with the medical team., • Refer patients or family members to community resources or to specialists as necessary., • Manage patient crises as needed.
