    General Medicine Physician – Geriatric (Primary Care) Salary: $250,000–$300,000 Base + Performance Bonus Locations: Davenport, FL | Zephyrhills, FL | Desoto, TX Schedule: Monday–Friday, 8:00 AM–5:00 PM | No Weekends | No On-Call Palm Medical Centers is seeking a patient-centered Primary Care Physician with a geriatric focus to join our growing, value-based care organization. Why Join Palm Medical Centers? • Competitive base salary + performance bonus, • Health, Dental & Vision Insurance, • Generous Paid Time Off, CME Time & Paid Holidays, • Employer-paid Life Insurance & Long-Term Disability, • 401(k) with company support, • Flexible Spending & Dependent Care Accounts, • Monday–Friday schedule (8–5), no weekends, no on-call, • Relocation assistance available, • Supportive leadership and team-based care model, • Provide comprehensive primary care to adult and senior patients in a Medicare Advantage model, • Conduct routine, preventive, and annual wellness visits, • Manage chronic conditions and acute care needs, • Order and review diagnostic tests and lab work, • Develop individualized care plans focused on outcomes and patient satisfaction, • Practice value-based care emphasizing prevention and reduced hospitalizations, • Collaborate with NPs, MAs, case managers, and specialists, • Monitor and address quality metrics (HEDIS, STAR ratings), • Document patient encounters accurately and timely in eClinicalWorks (eCW), • Participate in quality improvement initiatives and multidisciplinary meetings, • Educate patients and families on preventive care, medications, and healthy lifestyle choices, • Adhere to clinical protocols, compliance standards, and ethical practices, • M.D. or D.O. from an accredited medical institution, • Board Certified or Board Eligible in Internal Medicine or Family Medicine, • Active, unrestricted Florida Medical License (TX license for Desoto, TX role), • Active DEA license in good standing, • 2–3+ years of experience treating Medicare and/or Medicare Advantage patients preferred, • Experience in a value-based care environment strongly preferred, • Knowledge of HEDIS, STAR ratings, risk adjustment, and preventive care, • Strong communication and collaboration skills, • Comfortable with EMR systems (eCW experience a plus), • Primary Care / Internal Medicine / Geriatrics, • Day shift, in-person, • Travel up to 25% (preferred), • 401(k), • Dental insurance, • Health insurance, • Paid time off, • Professional development assistance, • Relocation assistance
