About the department: Bassett Medical Center's Intensive Care Unit is a 14-bed critical care unit, supporting the critical care and needs of a network. Being this vital support for our rural community, and a trauma hospital, we utilize advanced treatments that rival large medical centers. The Intensive Care unit is consistently hiring for Registered Nurses with various shift and position types available. What you'll do: The Registered Professional Nurse (RN), Acute Care in the Intensive Care unit is accountable for the delivery of safe patient care utilizing the nursing process. The RN assesses, plans, delivers, coordinates, integrates, and evaluates nursing care related to patient’s needs. The RN contributes to the support of patients through interdisciplinary teamwork. Essential Duties and Responsibilities: The role of a Care Manager is to be responsible for the overall coordination of services to meet the goals of the client/families focused plan of care. This is accomplished by building relationships with the caregiver(s) and care team and acting as a liaison to other agencies and ensuring they receive all necessary services and implementing the plan of care. Our aim is to provide the highest quality of care management services by complying with regulatory guidelines and meeting agency standards. • Assists families to plan and obtain services appropriate to the needs of the child and family., • Completes the Child and Adolescent Needs Assessment (CANS) to identify child’s medical, social, educational and psychological needs. The role of a Direct Support Professional (DSP) is to work with children and their families to provide resources and support, as well as strengthen daily living skills.
