Program Description: Funded by the New York City Department of Health and Mental Hygiene (NYC DOHMH), the Harlem Health Advocacy Partners (HHAP) program aims to improve the health of NYC residents by supporting access to health and social services to which they are entitled, providing health coaching to individuals to increase self-efficacy for healthy behavioral changes and disease management, building community capacity to seek and/or create healthy conditions and acceptable services through advocacy to government and other service providers, and providing Group Wellness Activities and high-quality educational workshops. Position Summary: PHS seeks an HHAP Program Manager to oversee the day-to-day efforts of this grant, including staff management, engagement efforts with partners, overseeing community advocacy and outreach efforts, and ensuring that health equity is promoted throughout all the program’s objectives. The Program Manager will report to the Director, Community Health and Nutrition Access (CHANA), and work closely with the HHAP coordination team that includes the Deputy Director, CHANA; Senior Program Manager, Food Initiatives; Senior Program Manager, Healthcare-Community Partnerships; CHW Supervisors, HHAP; and the Operations Coordinator, HHAP. Duties and Responsibilities: • Program Planning, Management, and Implementation – Oversee program management and work collaboratively with the HHAP coordination team to implement the program in East and Central Harlem., • Oversee strategy and activities for outreach and partner collaborations with the goal of promoting enrollment in the program., • Coordinate outreach activities and events, using the outreach workplan to guide activities., • Identify new outreach opportunities. Connect with stakeholders (property management office leaders, health care partners, community-based organizations, etc.) to organize outreach activities. Share and conduct follow-up as needed to ensure logistics are finalized., • Oversee the scheduling and assigning of outreach activities to CHWs., • Attend events hosted by the program in the community., • Meet regularly with community partners to discuss progress on outreach, new ideas and strategies, and additional needs such more materials, language needs, etc., • Work closely with, cultivate new and maintain existing collaborations with healthcare partners, community-based organization partners, and other community leaders/stakeholders in East Harlem with the goal of increasing partner referrals to the program., • Oversee Health Coaching/ One-on-One Assistance Implementation., • Work closely with the CHW Supervisors to oversee the implementation of Health Coaching and one-on-one education and referral support by CHWs., • Maintain accessible directory of local partner and referral resources for CHWs to access, and support CHWs to utilize Unite Us for in-bound and out-bound referrals., • If gaps in service needs are identified by CHWs, work with the HHAP team and partners to find solutions for gaps., • Oversee wellness workshop implementation., • Oversee process for scheduling and organizing wellness workshops at the developments, including CHW assignments and calendar., • Maintain up-to-date list of available workshop topics for CHWs to conduct at developments., • Review workshop requests from CHWs. Reach out to partners to confirm availability to conduct workshops or confirm internal capacity to present on the topic. Coordinate with CHWs to confirm scheduling., • Provide final sign-off on all internally developed workshop content and materials., • For new workshop topic requests, identify and reach out to partners with content experience to arrange new workshops or coordinate with the HHAP team to have the content developed internally., • Oversee Advocacy/ Campaign Activities Implementation., • Work closely with the Sr. Program Manager, Food Initiatives, to oversee implementation of advocacy and campaign activities., • Work with CHW Supervisors to manage and assign CHW roles in implementing advocacy campaigns., • Staff Supervision and Human Resources Management - Manage HHAP program staff oversight, recruitment, training, and ongoing supervision., • Provide direct supervision for Operations Coordinator and CHW Supervisors. Monitor staff performance and productivity and conduct regular supervision meetings., • Oversee performance management for the HHAP team and support CHW Supervisors to monitor CHW performance and adherence to the program’s protocols. Develop and implement process for field observation and work with Supervisors to address issues and gaps in training., • Complete annual performance review process and work with HR to handle any staff concerns as needed., • Oversee hiring and onboarding process for CHWs and Supervisors, including recruiting for open positions, onboarding new hires using customized checklists, implementing a structured shadowing process, and ensuring all necessary trainings for new hires are completed., • Oversee training for the HHAP team., • Manage the list of required and optional trainings for CHWs. Ensure any trainings that should be completed annually are scheduled throughout the year., • Identify and schedule training and train-the-trainer opportunities for the team to fill gaps as needed., • Create, maintain, organize, and update training materials as needed (including desk aids for key info, process flow charts, health coaching training, best practices)., • Quality Assurance and Maintenance – Monitor program data and oversee the tracking of program activities, ensuring quality of all data collected., • Regularly monitor program data in collaboration with the HHAP coordination and Quality and Evaluation teams, including the development and maintenance of Salesforce Dashboards., • Oversee the development of program materials and templates (such as event trackers, surveys, pre-screening form, sign-in sheets, etc.) and update as needed., • Oversee the collection and tracking of program activities on Forms, SharePoint, and Salesforce (event forms, leads, sign-in sheets, etc.) and ensure processes and protocols for tracking, storing, and quality control of program information., • Grant management – Support grant management of the program., • Support strategic workplans for the program, as required by DOHMH, including the Outreach Workplan and the Educational Workshops & Group Wellness Plan., • Support development of the Advocacy Workplan and quarterly advocacy reports in collaboration with the Senior Program Manager, Food Initiatives., • Draft monthly activity reports using DOHMH template., • Assemble supporting documentation required for the quarterly vouchers., • Funder and Partner Communication, • Act as primary contact for partners and NYCHA requests., • Act as a contact for NYC DOHMH and work closely with their team on strategy, data evaluation, and program implementation., • Represent the program in strategic planning meetings with DOHMH, NYCHA, other funded CBOs, and other relevant meetings., • Maintain current knowledge of community health, chronic disease management, and information relevant to the HHAP program., • Attend in-person meetings and events as needed., • Work successfully in the office and in the community, with some remote work expected., • Other duties as assigned by the program. Qualifications and Skills: • B.A. or B.S. degree or higher., • Minimum of 2-3 years’ experience in public health with coursework in community engagement and outreach, community health or related field preferred., • Experience working in East and Central Harlem and with NYCHA residents preferred., • Experience managing a program/ project and team., • Experience directly supervising staff (CHW supervision a plus)., • Ability to work on-site or in the Harlem Community at least 3 days per week.
