This is a Full-Time Position and you will be reporting directly to the program supervisor. Job Function: Assist Program Supervisor in managing the daily operations of a licensed residential program providing transitional housing and restorative services to people with psychiatric disabilities. Qualifications: BA in Human Services with one year of experience working with persons with psychiatric disabilities or a High School Diploma with 5 years of experience. Knowledge of residential services, evidence-based practices, and psychiatric rehabilitation practice. Clean, valid NYS Driver’s License. Job Description RiseWell Community Services is seeking a compassionate Full-Time Substance Abuse Counselor to join our team at our PROS program in Copiague, NY In this role, the Substance Abuse Counselor will provide therapeutic groups and individual interventions to assist members in growth and recovery. Responsible for substance abuse treatment in program. Work as part of an interdisciplinary team. Complete all required documentation within a 48 hour period and complies with program and OMH regulations. Requirements: CASAC preferred. Masters in Social Work or Mental Health Counseling or related field preferred. NYS licensed preferred. At least one year of experience in mental health and substance abuse treatment. Job Description Established in 1926, Family Service League is a social services agency transforming lives and communities through mental health, addiction, housing, and essential human services with more than 60 social service programs in over 20 locations. SCHEDULE: 19 hours weekly Hours/days to be determined SUMMARY Family Service League is seeking a part-time Case Manager for the Healthy Families Nassau program. The Case Manager will be responsible for engaging new referrals of prospective participants to determine eligibility for Healthy Families. The Case Manager will also conduct interviews and complete assessments with expectant parents or parents who have a child less than three months of age. The Case Manager will provide home visits to families located throughout Nassau County, but primarily in Hempstead, Uniondale, and Roosevelt. Travel is required. RESPONSIBILITIES • Conduct home visits to assess family needs and provide ongoing support to promote healthy family and parenting choices, child development, and positive parent-child relationships., • Assist with outreach activities in the program's target communities., • Conduct assessment in the participant’s home through a one-to-one conversational weave process with all eligible families during the prenatal or postpartum period., • Thoroughly document assessments and home visits in narrative form., • Participate in weekly reflective supervision to discuss assessments and home visits, collaborate on service planning, and review documentation., • Coordinate with other community services to support program participants and activities., • Attend and participate in all scheduled Healthy Families conferences, outreach, seminars, in-service training, and parent events., • Serve as an advocate for participants and potential participants as required by linking and/or referring families to appropriate community resources., • Plan and provide short term direct assistance if required with the supervisor’s approval., • Maintain tracking forms, required statistical information, data forms, and other paperwork as required, in a detailed and timely manner., • Maintain flexibility in scheduling (able to work limited nights and weekends)., • Participate in team activities and agency Continuous Quality Improvement activities., • Establish trusting relationships with families and work effectively with mothers, fathers, and extended family., • All other duties as assigned.
