Key Responsibilities • Assist clients with activities of daily living, including bathing, grooming, dressing, toileting, and personal hygiene., • Provide compassionate companionship and emotional support., • Assist with mobility, transfers, and ambulation according to the established care plan., • Prepare meals and assist with feeding as appropriate., • Perform light housekeeping, laundry, and approved household tasks., • Encourage client independence and participation in daily activities., • Observe and promptly report changes in client condition to the supervising nurse., • Maintain accurate documentation of care provided., • Follow infection prevention, safety, confidentiality, and agency policies., • Respect each client's spiritual beliefs and preferences, offering appropriate spiritual companionship when requested., • Maintain a professional, compassionate, and respectful environment in every client's home. Qualifications and Requirements • Active New Jersey Certified Homemaker-Home Health Aide (CHHA) certification required., • Previous home care, elderly care, or dementia care experience preferred., • Strong communication and interpersonal skills., • Compassionate, patient, trustworthy, and dependable., • Ability to follow individualized care plans and supervisory instructions., • Reliable transportation to assigned client locations., • Ability to meet applicable New Jersey background screening and employment requirements., • Commitment to client safety, confidentiality, and professional boundaries., • Willingness to uphold Holy Trinity Home Care Agency's mission and core values.
