Options for Community Living, Inc. is looking for a Supportive Services Case Manager/Health Educator to join our team! Schedule Options: (35 hours/week) • Monday - Friday: 8:00 AM - 4:00 PM Pay Type: Non-exempt Responsibilities: The Supportive Services Case Manager/Health Educator provides community-based services focused on linking, engaging and re-engaging persons with HIV/AIDS to care and treatment with an emphasis on frequent contact as a means of retaining the individual in care, avoiding health-related crises, and achieving self-management. Provides health education services to facilitate skills that encourage consumers to be responsible for their health care and lifestyle choices. The Supportive Services Case Manager/Health Educator conducts outreach to recruit eligible clients, assess/prioritize needs, reduce barriers and establish goals. Provides case management services in coordination with service providers. Assist with applying for insurance as necessary. Conduct consumer Health Education screenings and facilitate Health Education group process. The Supportive Services Case Manager/Health Educator assists with implementation of Health Education Curriculum, implementation of contract goals and preparation of necessary reports to funding sources. • Assist in the implementation, planning and coordination of program policies, procedures and operations., • Assist in the development, implementation, planning and coordination of program policies, procedures, and operations., • Conduct consumer screenings, documenting reasons for participation in Health Education services., • Maintain contact with referral sources, conduct outreach to identify program candidates., • Complete intakes and gather supporting documentation. Record appropriate information into Foothold Awards (EMR). Reassess program eligibility quarterly., • Assist Program Manager and Program Supervisor with development of HIV/AIDS health curricula and provide instruction including but not limited to understanding the importance of viral suppression; lab results; consequences of poor/sporadic medication adherence; nutrition; support networks; health literacy; goal setting; program solving strategies; stress reduction techniques; managing co-morbidities; and effective communication with medical providers., • Provide case management services to engage and maintain the consumer in care and improve health outcomes., • Provide advocacy services, and/or coordinate necessary transportation for clients., • Make regular contact with clients to ensure health and safety and access to services (weekly contact with min. one face-to-face contact monthly, promote monthly attendance in group health education). Must travel to visit clients using own vehicle., • Complete assessments, reassessments and service plans with additional health education goal(s) as needed., • Participate in case conferences (min. once every 90 days) and coordinate referrals for services and clinical providers to facilitate engagement and retention in care., • Work with Care Navigators to engage consumers in care and treatment and support services., • Crisis intervention as needed., • Coordinate program admissions, discharges, and case closures.
