About the Role: The Family Nurse Practitioner (FNP) plays a crucial role in delivering comprehensive healthcare services to patients across various age groups and backgrounds. This position involves assessing, diagnosing, and managing a wide range of health conditions, ensuring that patients receive high-quality care tailored to their individual needs. The FNP will collaborate with a multidisciplinary team to develop treatment plans, provide patient education, and promote wellness within the community. Additionally, the role includes performing procedures such as chemotherapy administration and pain management, while prioritizing patient safety and comfort. Ultimately, the FNP is dedicated to improving patient outcomes and enhancing the overall health of the populations served at TRUE HEALTH locations in Hicksville, Brooklyn, and Great Neck. Minimum Qualifications: • Master's degree in Nursing with a specialization as a Family Nurse Practitioner., • Current and valid Nurse Practitioner license in the state of New York., • Experience in a primary care or urgent care setting., • Familiarity with electronic health record (EHR) systems., • Bilingual skills to effectively communicate with diverse patient populations., • Minimum 1 year experience as NP, • Graduated from approved Nurse Practitioner Program., • Licensed as Nurse Practitioner in New York State., • Basic CPR (BCLS) certification required., • Specialty Certification Preferred., • Conduct comprehensive health assessments and physical examinations for patients of all ages., • Diagnose and manage acute and chronic health conditions, including prescribing medications and treatments., • Educate patients and their families on health management, disease prevention, and wellness strategies., • Collaborate with healthcare team members to develop and implement individualized care plans., • Perform necessary procedures, including chemotherapy administration and urgent care interventions, while ensuring patient safety., • Involved with direct patient physical assessment, education, and health planning., • Assists in the coordination of services with other disciplines and patient’s insurance plans., • Advocates for patient in arrangement of medically necessary services., • Communicates with community’s resources to ascertain adequate assistance for patients in need., • Refers patients to specialty services when necessary., • Consults with collaborating physician and other members of the health care team to establish plan of care., • Participate in LEAN and other activities necessary to implement the multi-disciplinary patient-centered medical home approach for the care of patients, • Maintains continuity of care by regular patient follow-up., • Participates in quality assurance process to assure optimal care delivery., • Provides comprehensive case management services that include emphasis on health promotion and disease prevention., • Monitors patient outcome in accordance with the practice protocols;, • Collaborates with staff nurses and nursing administrators with assessment and clinical decision in patient care planning., • Acts as a resource person for the nursing staff., • Conducts education in service on practice issues. The required skills of a Family Nurse Practitioner are essential for providing high-quality patient care on a daily basis. Proficiency in patient education allows the FNP to empower individuals to take charge of their health, while expertise in chemotherapy and pain management ensures that patients receive appropriate treatment for complex conditions. Critical care skills are utilized in urgent situations, enabling the FNP to respond effectively to emergencies. Strong communication and interpersonal skills foster collaboration with colleagues and build trust with patients, enhancing the overall care experience. ESSENTIAL JOB DUTIES AND RESPONSIBILITIESAdministrative (10%):•Participates in department and interdisciplinary committees which affect policies related to the role of the Advance Practice Clinician and delivery of health care to the customer.•Participates in agency quality risk management program. Under the direction of the regional VP and the physician leadership, implements overall quality assurance activities at the health centers•Responds with other staff to community needs by participating in educational efforts.•As required by law, participates in Collaborative Agreement with Medical Director or another designated Physician.•Accepts individual and joint responsibility for and participates in health care center efforts to comply with all organizational and quality guidelines and all pertinent government regulations.•Understands and commits to customer satisfaction approach to health care delivery.•Accepts individual and joint responsibility for and participates in overall health care center efforts to achieve all goals set to improve customer relations, increase customer satisfaction and promote repeat customer visits.•Complete all responsibilities according to established protocols, policies and standard practices in the areas of ADA and regulatory compliance programs such as HIPAA.•Works effectively with volunteers, trainees and temp agency personnel.•Performs other educational and experience related duties as required.•Adhere to affiliate goals, policies and standards of conduct in terms of professionalism. Assist center staff in achieving time efficient family planning visits as well as answering phones professionally.•Familiar with health center emergency procedures and responds appropriately.•Monitors and ensures compliance with OSHA, CLIA and State procedures, policies and regulations.•Prepares medical incident / occurrence reports as needed.•Travel: While performing the duties of this job, the employee may have to travel toother work locations at various times. Own transportation is required, with a valid driver’s license, inspection, registration and adequate insurance. Direct Clinical Care: (80%)•Responsible for assessing, planning and administering health care which is individualized to the patient and consistent with the policies and protocols of PPFA and PPGNY.•Secures a complete health history and performs physical examinations with special emphasis on the reproductive health care, and other types of more specialized reproductive care including abortion care, and transgender hormone therapy.•Provides medications and contraceptive devices as needed.•Monitors medical aspects of all medical services offered at the health center, acting as a resource to center staff and patients regarding medical concerns.•Performs, orders and interprets appropriate laboratory tests.•Provides education and appropriate management for women and men in need of reproductive related services, including fertility control, infertility, and sexually transmitted infections.•Assesses and manages gynecological conditions and provides treatment consistent with PPGNY protocols, policies, and procedures.•Ensures appropriate medical follow-up on all patient testing and/or treatment, including direct patient contacts and laboratory test review.•Create & maintain medical documentation in an EHR system in compliance with EHR policies and procedures.•Provides postpartum care in selected settings within an interdisciplinary team framework with appropriate pre-service and in-service education.•Seeks consultations as necessary from Medical Director or designee.•Collaborates with physicians and other members of the health care team and makes referrals to medical and community resources as indicated. Training: (10%)•At the discretion and direction of the Clinical Director, AVP of clinical implementation and or Medical Director, assists in training and orientation for newly hired APC’s and new skill development.•At the discretion and direction of the health center management, assists in training and orientation for newly hired clinical support staff.•Attends agency sponsored staff meetings, medical in-services and trainings.
