Job Description Motor Assistance Vehicle Technician (MAVT)/ Motor Assistance Vehicle Operator (MAVO) SUMMARY: The Motor Assistance Vehicle Technician/Operator (MAVT/MAVO) is responsible for all aspects of patients’ care within the scope of certification. Essential Job Functions The Motor Assistance Vehicle Technician/Operator (MAVT/MAVO) is an integral part of the Virgo transportation team. In this position the MAVT/MAVO transports ambulatory patients to and from residences and or skilled nursing facilities to medical treatments and or appointments, and then return them. Some of the patients are able to ambulate to our vehicles while other must be moved in their wheelchairs, or are able to move on their electric scooters. No person is to ride the lift while standing, or on the electric scooter. The patients are placed in a wheelchair to go up on the lift, and they are moved inside and secured to the vehicle either on the bench seat or in the wheelchair. Should there be an aide or family member accompanying the patient, they must be secured in the vehicle as well. The MAVT/MAVO must check to see if any belongings or paperwork must accompany the patient for the transport, and then ensure that it gets dropped off with the patient. For each trip the appropriate documentation must be filled out, and where required, obtain the necessary signatures from the patient and /or facility. In the event monies are to be collected at time of service, a receipt is to be filled out and a copy given to the patient, or the person accompanying the patient. The MAVT/MAVO must be capable of good oral communications as for every leg of the trip dispatch must be communicated with, as to alert them to the status of the transport. In addition should any incident occur dispatch must be notified in a timely manner. The vehicle being used for any particular shift must be checked both prior and post usage for mechanical operations, fluids, and general cleanliness. The MAVT/MAVO must be able to utilize maps in order to locate facilities or residences that they are not aware of, with the assistance of dispatch. At the conclusion of the MAVT’s/MAVO's shift they must turn in the appropriate paperwork, and monies collected. Saint James Health Community Health Center (SJH) offers comprehensive, community-based, preventive, primary care, and wellness services for New Jersey residents of all ages at three convenient locations in Newark, NJ. Health care services are designed to meet the specific needs of the entire family - from annual check-ups and immunizations to sick visits and mental health counseling. The Licensed Clinical Social Worker (LCSW) is responsible for the implementation of the Social Work Department Strategic Plan. The LCSW will work in an environment that supports clinical excellence and competency through multi-disciplinary teamwork and collaboration. The LCSW will develop a plan for social work intervention that is consistent and congruent with the medical treatment, considering the patient's overall psychosocial status, physical and emotional needs. The LCSW will establish and maintain a therapeutic relationship with the patient, family and/or significant others and with their participation, formulates treatment goals that address identified needs or problems and a plan of action for achievement of these goals. Responsibilities: • Provides social services and assistance to improve the physical, social and psychological functioning of patients and their families., • Identifies psychosocial status, economic and physical needs of patients; assesses and reassesses patient's support systems, available community resources and other factors to plan, develop, and implement an appropriate plan of care., • Provides psychosocial assessments, intervention, and care plan to patients of the Health Center and assists them and their families in coping with their illness, diagnosis, and/or life situation and meet the continuing health care needs., • Identifies emotional or behavioral issues and diagnose disorders in accordance with ICD-10 codes, using information obtained from screening tools, interviews, records and reference materials., • Counsel individuals and groups (if applicable) in order to modify behavior to improve personal, social and vocational adjustment., • Develop and implement individual treatment plans specifying type, frequency and duration of therapy, • Operates as an advocate with all community populations utilizing skills that respect issues of cultural and ethnic diversity and equity for every patient and family, • Coordinates services to Health Center, community agencies and other professionals; maintains working knowledge of and liaison with community agencies; participates in area activities related to health and social services., • Assumes responsibility for his or her own continued professional development; keeps current with social work and health care developments and seeks to increase further enhancement of job related knowledge., • Collaborates with the health care team and involves the patient/family in the development and implementation of plans., • Facilitates patient care across a multidisciplinary care continuum which includes interacting with Clinic Providers, and other health care providers to facilitate movement of the patient through the system and arrangement of an appropriate plan of care., • Serving as a patient advocate, maintains a working knowledge of relevant medical/legal issues that have an impact on patient care, e.g., advance directives, child and elder abuse., • Utilizing the EMR system, maintains records and statistics in accordance with department policies., • Attends and participates in staff committees, departmental clinical collaboratives and other administrative meetings., • Contributes to the efficiency and effectiveness of the Health Center's service to patients by offering suggestions and directing or participating as an active member of the health care team., • Works collaboratively with all members of the Senior Management and Management Teams to resolve departmental and organizational challenges expeditiously, economically, professionally and with minimum disruption to the practice., • Constantly strives to achieve excellence in day-to-day operations of respective department., • Completes all mandatory in-services and attends required meetings., • Adheres to Corporate Compliance policies., • Is highly accountable for one's own productivity.
